Acute Abdominal Incidens in Prehospital Emergency Care

Abstract:

Cílem bakalářské práce je deskriptivní metodou přiblížit náhlé příhody břišní a postupy jejich řešení v kontextu přednemocniční neodkladné péče. Po seznámení s definicí přednemocniční neodkladné péče a jejím ukotvení v legislativě České republiky konkrétněji poukazuje na anatomii dutiny břišní. Poté se věnuje charakteristice náhlých příhod břišních a jejich primárnímu rozdělení z hlediska mechanizmu vzniku. Podrobněji se zabývá neúrazovými náhlými příhodami břišními a jejich rozlišením z aspektu existence zánětu, ileu, krvácení, také uvádí náhlé příhody břišní gynekologického a urologického původu. Dále se zaměřuje na úrazové náhlé příhody břišní a jejich diferenciaci z lediska přítomnosti či nepřítomnosti penetrujícího poranění. Součástí teoretické části je také rámcový popis potenciálních diagnostických a terapeutických možností náhlých příhod břišních v možnostech přednemocniční neodkladné péče. V praktické části jsou uvedeny kazuistiky několika klientů s náhlou příhodou břišní vzniklou úrazovým i neúrazovým mechanizmem. Součástí kazuistik je komplexní průběh zásahu, počínaje převzetím počátečního apelu dispečinkem záchranné služby, určením typu výjezdové posádky, výjezdem sanity, přes kontaktování klienta, popřípadě jeho okolí. Dále se věnuje reálným diagnostickým a terapeutickým možnostem ve skutečných podmínkách přednemocniční neodkladné péče a možným komplikacím. Pokračuje průběhem transportu až ke konečnému předání klienta zdravotnickému zařízení. Nakonec ověřuje správnost uskutečněných diagnostických a terapeutických postupů ve srovnání s doporučenými postupy uváděnými v literatuře. Závěrem se zaměřuje na statistické skutečnosti typu posouzení množství výjezdů posádek záchranné zdravotnické služby k náhlým příhodám břišním z hlediska celkového počtu apelů, taktéž z aspektu mechanismu vzniku, pohlaví nebo věku v podmínkách Zdravotnické záchranné služby Olomouckého kraje.

Abstract:

The objectives of the bachelor thesis: To introduce the topic of acute abdominal emergency and methods of its treatment in the context of urgent care. After explaining the definition of pre-hospital delivery urgent care and its embedding in the legislation of the Czech Republic the work deals with the anatomy of abdominal cavity in a more specific way. The following part presents characteristics of acute abdominal emergency and its primary dividing from the point of view of the mechanism of its origins. It describes more in detail non-accidental acute abdominal emergencies and their distinction from the aspects of the existence of inflammation, ileus, bleeding and also acute abdominal emergency of gynaecological and urological origins are mentioned here. The following part is aimed at accidental acute abdominal emergencies and their differentiation according to the presence or absence of penetrating injuries. General description of potential diagnostic and therapeutical possibilities of acute abdominal emergencies in the frames of pre-hospital delivery urgent care is mentioned in the theoretical part. In the practical part are given casuistries of several clients suffering from acute abdominal emergencies caused by accidental as well as non-accidental mechanisms. One part of the casuistries indicates also a complex history of the intervention beginning with the acceptance of the call by the dispatching of the medical emergency service, assignation of the type of the ambulance car crew, departure of the ambulance car and contacting the client or his surroundings. Further on it deals with real diagnostic and therapeutical possibilities in real conditions of pre-hospital delivery urgent care and possible complications. It continues with the transport to the final delivery of the client to the health care institution. Finally it verifies the correctness of the realized diagnostic and therapeutical proceedings in comparison with recommended proceedings indicated in literature. In conclusion of the work are presented statistical facts connected with the examination of the number of departures of emergency ambulance health care crews in connection with acute abdominal emergencies in the context of all departures and also from the point of view of the origins, genders or age in conditions of Emergency Medical Service of Olomouc region.
