Abstract:

Bakalářskou práci píši na téma Validace ošetřovatelských prvků u ošetřovatelské diagnózy: Neefektivní kojení 00104. Zajímám se především o klasifikaci NANDA v českých podmínkách. Hlavním cílem práce bylo validovat diagnostické prvky ošetřovatelské diagnózy Neefektivní kojení 00104 podle vybraných sester expertek a zjistit, které prvky považují za hlavní (major) a které za vedlejší (minor). Použila jsem obsahovou validaci podle Fehringa, DCV model (Diagnostic Content Validity). První dotazníkové anonymní šetření vyplňovalo 29 sester expertek (porodní asistentky i dětské sestry), které jsou v praxi alespoň 5 a více let, které pracují ve vybraných nemocnicích Ústeckého kraje. Měřícím nástrojem byly použity diagnostické prvky, které jsou použity v klasifikačním systému NANDA-International. V diagnóze Neefektivní kojení 00104 považují sestry expertky za významné určující znaky (vážené skóre vyšší než 0,80): kojenec ztrácí hmotnost (0,87) a kojenec nepřibývá na váze (0,84). Jako významné související faktory byly označeny: úzkost matky (0,84) a předčasně narozené dítě (0,82). Sestry expertky tak považují za významné 4 prvky z 30. Vážený průměr všech diagnostických prvků vychází na 0,69. Druhé anonymní dotazníkové šetření vyplňovalo 30 maminek respondentek. Sběr dat probíhal na vybraných pracovištích Ústeckého kraje. Byly zde kladeny uzavřené otázky. Z výsledků je zřejmé, že informace o kojení, které matkám sestry poskytují v průběhu hospitalizace na oddělení šestinedělí, jsou nedostačující. Matky nemají základní dostatečné informace o kojení.

Abstract:

I write this bachelor thesis on the Validation of Nursing Elements of a Nursing Diagnosis: the Ineffective Breastfeeding 00104. I am interested especially in the NANDA classification in the Czech environment. The main aim of this thesis is to validate the diagnostic elements of the nursing diagnoses the Ineffective breastfeeding 00104 according to chosen expert nurses and to find out which elements they regard as major and which they regard as minor. I used the Fehring´s model of Diagnostic Content Validy (DVC model). The first anonymous questionnaire was filled in by 29 expert nurses (midviwes and pediatric nurses) with the working experience of 5 years and longer and who work in chosen hospitals in Ústí nad Labem Region. As the measuring tool the diagnostic elements from the NANDA-International classification system were used. In the diagnosis of the Ineffective breastfeeding 00104 the expert nurses consider as significant features (weighted scores highter than 0,80): the infant loses weight (0,87) and the infant does not gain weight. Other related significant factors that were identified: maternal anxiety (0,84) and premature baby (0,82). The expert nurses consider as significant 4 elements out of 30. The weighted avarage of all the diagnostic elements is 0,69. The second anonymous questionnaire was filled in by 30 mothers respondents. The data collection was carried out in chosen workplaces in Ústí nad Labem Region. Closed questions were used here. It is clear from the results that the information about breastfeeding that nurses provide to mothers during hospitalization at the puerperium department is insufficient. There are differences between the opinions of nurses and mothers on the breastfeeding education.
