Abstract:

Úvod: Sluch je jeden z našich nejdůležitějších smyslů. V současné době jsme již od dětství vystavování hluku a tím dochází k rychlejšímu zvyšování sluchového prahu a postupnému zhoršení sluchu. Intervence v této oblasti zatím není zařazena do preventivních programů ve vzdělávacích zařízeních. Cíl: Hlavním cílem diplomové práce bylo zhodnocení sluchového prahu u vysokoškolských studentů Lékařské fakulty spolu se subjektivním hodnocením expozice hluku během volnočasových aktivit. Metodika: V prvním roce bylo provedeno audiometrické vyšetření u 100 studentů, 1. ročníku Všeobecného lékařství v rámci výuky vyšetření sluchu pomocí tónové prahové audiometrie s vyhodnocením celkové procentuální ztráty sluchu dle Fowlera, spolu s dotazníkovým šetřením. Ve druhém roce, byl rozeslán dalším 250 studentům různých zdravotnických oborů pouze dotazník s návratností 34,4 %. Hodnocení dat následně proběhlo celkem u 186 studentů ve věkovém rozmezí 19-25 let, resp. 141 žen a 45 mužů, včetně vyšetřovaných 100 studentů tónovou prahovou audiometrií. Pro statistickou analýzu byla zvolena 5 % hladina významnosti. Výsledky: Sledovaný soubor se neprokázal zvýšením sluchového prahu z důvodu expozice hluku ve volném čase zejména prostřednictvím používání sluchátek k poslechu hudby i přesto, že více než 90 % studentů sluchátka soustavně používá. Dotazníkové šetření prokázalo u mnohých sledovaných parametrů statisticky významné rozdíly a to např. u používání a frekvence používání sluchátek při srovnání dle pohlaví, nebo v době používání sluchátek při jednom použití a výskytu sluchové únavy po návštěvě hudební akce. Kladná korelace byla nalezena pouze mezi průměrnými sluchovými ztrátami na řečových frekvencích (1000-4000 Hz) a začátkem používání sluchátek a mezi ztrátami sluchu na frekvenci 8000 Hz a preferovaným stupněm hlasitosti při poslechu hudby přes sluchátka. Závěr: Diplomová práce nepotvrdila vliv používání sluchátek na zhoršení sluchu v podobě zvýšení sluchového prahu u sledovaných studentů. Nicméně díky prokázání tohoto negativního vlivu na zhoršení sluchu v zahraničních studiích, je doporučeno všem studentům a obecně mladé populaci dbát na vhodnou prevenci při volnočasové expozici hluku.

Abstract:

Introduction: Hearing is one of our most important senses. At present, we have been exposed to noise since childhood, and this leads to a faster increase in the hearing threshold and a gradual deterioration of hearing. Interventions in this area are not yet included in prevention programs in educational institutions. Aim: The main aim of the diploma thesis was to evaluate the hearing threshold of university students of the Faculty of Medicine together with the subjectively evaluation of noise exposure during leisure activities. Methodology: In the first year, an audiometric examination was performed on 100 students, 1st year of General Medicine within the teaching of hearing examination using tone threshold audiometry with evaluation of the total percentage of hearing loss according to Fowler, together with a questionnaire survey. In the second year, only a questionnaire with a return of 34.4 % was sent to another 250 students of various medical disciplines. Data evaluation was subsequently performed on a total of 186 students in the age range of 19-25 years, resp. 141 women and 45 men, including 100 students examined by tone threshold audiometry. A 5 % level of significance was chosen for statistical analysis. Results: The sample did not show an increase in the hearing threshold due to exposure to noise in leisure time, mainly through the use of headphones to listen to music, even though more than 90 % of students use headphones consistently. The questionnaire survey showed statistically significant differences in many monitored parameters, such as the use and frequency of using headphones when compared by gender, or at the time of using headphones in one use and the occurrence of hearing fatigue after attending a music event. A positive correlation was found only between the average hearing loss at speech frequencies (1000-4000 Hz) and the beginning of the use of headphones and between the hearing loss at 8000 Hz and the preferred volume level when listening to music through headphones. Conclusion: The diploma thesis did not confirm the effect of using headphones on hearing impairment in the form of increasing the hearing threshold in the monitored students. However, due to the demonstration of this negative effect on hearing impairment in foreign studies, it is recommended that all students and the young population in general pay attention to appropriate prevention during leisure noise exposure.
