Effective acute pain management for inpatients

Abstract:

Adekvátní léčba akutní bolesti u hospitalizovaných nemocných. Neadekvátní léčba pooperační bolesti vede k celé řadě patofyziologických důsledků, z nichž mnohé zhoršují celkovou morbiditu a významně protrahují rekonvalescenci po chirurgických výkonech. Riziko vzniku pooperační persistující bolesti rovněž stoupá. Základní organizační jednotkou v léčbě pooperační bolesti je Acute Pain Service. Hlavní spektrum analgetické péče je zaměřeno na optimalizaci farmakoterapie a invazivní anesteziologické techniky. Ve výzkumné části jsme sledovali analgetický efekt etorikoxibu podaného perorálně před tonzilektomií. Prokázali jsme nižší spotřebu neopioidní pooperační analgezie, lepší kvalitativní složku a relativně stabilní intenzitu bolesti v etorikoxibové skupině. Kvalitu pooperační epidurální analgezie a výskyt včasných komplikací jsme sledovali u pacientů prvních 24 hodin po rozsáhlých výkonech. Konstatujeme výsledky v kvalitě dosažené epidurální analgezie srovnatelné s dostupnými metaanalýzy. Výskyt včasných komplikací je uspokojivý a zavážné neurologické komplikace nebyly shledány. Vybudování konceptu, legislativy a zajištění klinické dostupnosti Acute Pain Service ve Fakultní nemocnici u sv. Anny bylo třetím cílem. Léčba akutní pooperační bolesti je integrální součástí perioperační medicíny. Dostupnost pro každého pacienta na nejvyšší možné analgetické úrovni by měla být standardem.

Abstract:

Effective acute pain management for inpatients. Inadequate postoperative pain management leads to a number of pathophysiological consequences, which can worse postoperative morbidity and prolong recovery after surgery. The risk of postoperative persistent pain is also increase. The basic organizational unit in the postoperative pain management is Acute Pain Service. The main spectrum of postoperative analgesia is focused to optimizing pharmacotherapy and invasive anesthetic techniques. The research part was focused on analgesic effect of etoricoxib administered orally before tonsillectomy. We found a lower consumption of non-opioid postoperative analgesia, better quality and relatively stable component of the intensity of acute pain in etoricoxib group. The quality of postoperative epidural analgesia and the incidence of early complications were observed within 24 hours after major surgery. We found the results achieved in the quality of epidural analgesia comparable with the recent meta-analysis. The incidence of early complications is satisfactory and severe neurological complications were not found. Building concept, legislation and ensuring the availability of Acute Pain Service at the University Hospital of St. Anna was the third goal. Acute postoperative pain management is an integral part of perioperative medicine. Availability of effective postoperative analgesia at the highest possible level should be standard for each patient.
