Assessing the quality of life of seniors with spinal and spinal cord injuries

Abstract:

Cíl: Cílem výzkumu bylo zjistit kvalitu života seniorů po prodělaném úrazu páteře a míchy. Dílčí cíle byly zaměřeny na rozdíly v hodnocení kvality života po prodělaném úrazu páteře a míchy v jednotlivých doménách z hlediska doby po úrazu, z hlediska pohlaví, věku a míry soběstačnosti. Metodika: Výzkumné šetření probíhalo ve třech Rehabilitačních ústavech, od listopadu 2017 do prosince 2017. Do výzkumného vzorku bylo zařazeno 63 respondentů aktuálně hospitalizovaných na spinálních rehabilitačních jednotkách. Ke zjištění požadovaných cílů byla použita metoda kvantitativního šetření s použitím dotazníků zahrnující demografické údaje a údaje vztahující se k problematice míšního poranění. Ke zhodnocení míry soběstačnosti byl použit Barthelův test základních všedních činností a standardizovaný dotazník Světové zdravotnické organizace pro měření kvality života WHOQOL-BREF. Na analýze dat bylo spolupracováno s RNDr. Petrem Bujokem, Ph.D. Výsledky: Dotazníkovým šetřením bylo zjištěno, že poranění páteře a míchy se naprosto promítlo do života seniorů a ovlivnilo všechny oblasti postižených. Z šetření dále vyplynulo, že čím hůře hodnotí senioři kvalitu svého života, tím i hůře vnímají spokojenost se svým zdravím. Dalším zjištěním bylo, že kvalita života seniorů s úrazy páteře a míchy se liší v jednotlivých doménách z hlediska doby po úrazu, z hlediska pohlaví, věku a míry soběstačnosti. Z hlediska doby po úrazu kvalita života není závislá na době po úrazu. Dle porovnání se zahraničními i českými studiemi a statistikami je patrné, že lepší kvalitu života prožívají senioři s delším časovým odstupem od úrazu. Z hlediska hodnocení kvality života v závislosti na pohlaví, není kvalita života na pohlaví závislá. Z předešlých výzkumů je však patrné, že lépe je kvalita života hodnocena mužským pohlavím. Z hlediska věku je celková kvalita života lépe hodnocena mladšími seniory ve věkovém rozmezí 65-74 let, tuto skutečnost potvrzují i ostatní výzkumy a studie. Z hlediska míry soběstačnosti bylo prokázáno, že vyšší míra soběstačnosti zvyšuje kvalitu života seniorů. Závěr: Závěrem můžeme říci, že pacienti s úrazy páteře a míchy vyžadují vysoké psychické, sociální, fyzické a ekonomické požadavky na udržení kvality života, tak jak již potvrzují předešlé výsledky výzkumů.

Abstract:

Aim: The aim of the research was to determine the quality of seniors' lives after a spine and spinal cord injury. Partial targets were focused on differences in quality of life after spine injury and spinal cord injury in individual domains in terms of post-accident time, gender, age and self-sufficiency. Methodology: The research was carried out in three Rehabilitation institutions, from November 2017 to December 2017. The survey included 63 respondents currently hospitalized in spinal rehabilitation units. To determine the desired goals, a quantitative survey method using questionnaires including demographic data and data on spinal cord injury were used. To assess the degree of self-sufficiency, Barthel's index of Activities of Daily Living (ADL) and a standardized World Health Organization WHOQOL-BREF questionnaire were used. Data analysis was co-operated with RNDr. Petr Bujok, PhD. Results: A questionnaire survey found that spine injuries and spinal cord injuries were completely reflected in the lives of seniors and affected all areas. The survey further revealed that the worse the seniors assess the quality of their lives, the worse they perceive satisfaction with their health. Further findings showed that the quality of life of seniors with spinal and spinal cord injuries differs in individual domains from time to time after injury in terms of gender, age and self-sufficiency. In terms of post-accident time, quality of life is not dependent on post-accident time. Comparing with foreign and Czech studies and statistics, it appears that a better quality of life is experienced by seniors with a longer time span after accident. In terms of gender-based quality of life assessment, quality of life is not dependent on gender. However, previous research shows that the quality of life is better assessed by male gender. In terms of age, overall quality of life is better assessed by younger seniors aged 65-74, as is confirmed by other research and studies. In terms of self-sufficiency, it has been shown that a higher degree of self-sufficiency increases the quality of seniors' lives. Conclusion: In conclusion we can say that patients with spine and spinal cord injuries require high psychological, social, physical and economic requirements to maintain the quality of life, as already confirmed by previous research results.
