Frequency and nature of chromosome abnormalities in human IVF embryos

Abstract:

Chromozomové abnormality jsou hlavním důvodem zastavení vývoje embrya, selhání implantace, spontánních abortů a narození postiženého dítěte. Molekulární genetické analýzy odhalily, že nejčastěji vznikají v průběhu oogeneze, a že počet těchto chyb výrazně narůstá s věkem ženy. Asistovaná reprodukce s preimplantační genetickou diagnostikou a screeningem nabízí šanci počít zdravé dítě lidem, kteří by jinak tuto možnost nikdy mít nemuseli. V této práci se zabýváme četností, charakterem a klinickým významem chromozomových abnormalit u embryí páru, které podstupili léčbu neplodnosti v klinice asistované reprodukce Sanatorium Repromeda v Brně. Pro účely této práce jsme analyzovali výsledky vyšetření PGS a PGD u téměř 5000 embryí od více než 1000 párů. Většinový podíl (62 %) biopsií blastomer vykazoval chromozomou abnormalitu. U biopsií trofektodermu jsme pozorovali téměř poloviční množství aberantních embryí (44 %). Nejčastěji postižené chromozomy byly 15,16,21,22, X a Y. Výrazný nárůst aneuploidií u embryí byl pozorován u skupin žen nad 38 let. U PGD translokací jsme pozorovali výrazné snížení celkového zastoupení euploidních embryí a to především u skupiny biopsií blastomer (14 %). Jednotlivé segregační varianty u translokovaných chromozomů se lišily v závislosti na pohlaví přenašeče.

Abstract:

Chromosomal abnormalities are the main reason for developmental arrest of the embryo, implantation failure, spontaneous miscarriages and a birth of an affected child. Molecular genetic analysis revealed that chromosome abnormalities frequently arise during female meiosis and the incidence of errors increases rapidly with advanced maternal age. Assisted reproduction with preimplantation genetic diagnosis and screening helps plenty of couples suffering from infertility to conceive and give birth to a healthy child. In this thesis we discuss the frequency, nature and clinical importance of chromosomal abrnomalities in embryos of couples who underwent infertility treatment in the clinic of assisted reproduction Sanatorium Repromeda in Brno. For the purpose of this thesis we analyzed PGS and PGD results from almost 5000 embryos derived from more than 1000 couples. Majority of blastomere biopsies (60 %) was chromosomally abnormal. Concerning trophectoderm biopsies, nearly half of them (44 %) displayed chromosome anomalies. Most frequently affected chromosomes were chromosome 15, 16, 21, 22, X and Y. Significant increase in aneuploidy rate in embryos was observed in a group of woman older than 38 years old. Most notably, only 14 % of blastomere biopsies analysed for PGD of reciprocal translocation were chromosomally normal. We have also noticed that segregation pattern of translocated chromosomes was biased by the sex of the carrier.
