Identifikace a studium funkce mikroRNA s prediktivním a prognostickým významem u pacientů s kolorektálním karcinomem

Identification and functional studies of microRNAs with predictive and prognostic significance in patients with colorectal cancer

Abstract:

Východisko: Kolorektální karcinom patří mezi nejčetnější nádorová onemocnění v České republice a je druhou nejčastější příčinou nádorových úmrtí v západních zemích. Jedním z hlavních důvodů vysoké mortality je nedostatečná klasifikace. Současné studie proto usilují o nalezení nových specifických biomarkerů, které by umožnily spolehlivě odlišit jednotlivá klinická stádia tohoto onemocnění. Jednou z možných variant se jeví využití rozdílných expresních profilů miRNA. Metody: S využitím TaqMan® Array Human MicroRNA Set Cards v2.0. byly analyzovány expresní profily 667 miRNA u 63 pacientů s CRC a u 8 zdravých střevních sliznic. Exprese všech miRNA byla normalizována pomocí referenčního genu RNU48 a data byla dále zpracována pomocí standardních i vícedimenzionálních biostatistických metod v prostředí R (projekt Bioconductor, statistický balíček LIMMA). Součástí studie byly též in vitro funkční analýzy, jejichž cílem bylo optimalizovat tranzientní transfekci pre-miRNA do buněčné linie DLD-1 s využitím LipofectamineTM 2000. Pomocí MTS testu byl dále sledován vliv této transfekce na viabilitu buněk. Výsledky: S využitím statistické metody LIMMA bylo možné odlišit nádorovou tkáň od zdravé střevní sliznice na základě rozdílné exprese 65 miRNA (p = 0,001). Po uplatnění přístupu LOOCV byl získán set 26 miRNA, na základě kterého bylo možné odlišit pacienty ve II. a ve III. klinickém stádiu se senzitivitou 89 % a specificitou 77 %. Po optimalizaci transfekce bylo možné sledovat maximální nárůst exprese miR-145 a miR 378 v buňkách 24 hodin po transfekci 5 nM pre-miR-145/pre-miR-378. Průměrné ∆∆CT mezi kontrolními buňkami a buňkami transfekovanými bylo v případě miR 145 13,84 ± 0,44 cyklu, v případě miR-378 potom 9,86 ± 0,06 cyklu. MTS test bohužel neprokázal významný vliv zvýšené hladiny těchto miRNA na viabilitu buněk. Závěr: Studie potvrdila, že na základě rozdílných expresních profilů miRNA je možné spolehlivě odlišit nádorovou tkáň od zdravé střevní sliznice a zároveň umožnila identifikaci několika miRNA, které jsou specifické pro jednotlivá klinická stádia CRC. Takto nalezené miRNA bude nutné dále validovat na větším souboru pacientů. In vitro funkční analýzy prokázaly, že pomocí transfekce je možné regulovat hladinu miRNA v buňkách. Pro další výzkum však bude nezbytné nahradit tranzientní transfekci nádorových buněk transfekcí trvalou a též bude třeba sledovat vliv transfekce na další buněčné procesy, jakými jsou apoptóza, proliferace, buněčný cyklus či metastázování.

Abstract:

Objectives: Colorectal cancer (CRC) is one of the most common types of cancers in the Czech Republic and it is the second leading cause of cancer-related deaths in the Western countries. One of the main reasons for high mortality is insufficient classification. That is the reason why several efforts have been made to find new biomarkers in order to differentiate particular clinical stages of this disease. One possible way is to use different expression profiles of microRNA (miRNA). Methods: To investigate the expression profile of 667 miRNA in 63 colorectal cancer samples and in 8 adjacent normal tissue samples we used TaqMan® Array Human MicroRNA Set Cards v2.0. Expression of miRNA was normalized using the expression of the housekeeping gene RNU48 and data was statistically analyzed using R (project Bioconductor, LIMMA). In vitro functional analyses enabled us to optimize transient transfection of pre-miRNA into DLD-1 cells using LipofectamineTM 2000. Furthermore the effect of miRNA transfection on cell viability was measured by the MTS assay. Results: We were able to differentiate between adjacent normal tissue and colorectal cancer tissue using the different expression profile of 65 miRNA (p = 0.001). Using LOOCV technique we gained a set of 26 miRNA that enabled us to distinguish patients in II. and III. clinical stage with sensitivity 89 % and specificity 77 %. The maximum expression level of miR-145 and miR-378 was observed after 24 hours in cells transfected with 5 nM pre-miR-145/pre-miR-378. Average ∆∆CT between transfected cells and untransfected control was 13.84 ± 0.44 concerning the miR-145. In the case of miR 378, ∆∆CT was only 9.86 ± 0.06. Unfortunately there was no difference in viability between transfected and untransfected cells. Conclusion: In this study we confirmed that it is possible to differentiate between adjacent normal tissue and colorectal cancer tissue using the different expression profile of miRNA. Furthermore we were able to identify several miRNA with specific expression in particular clinical stages of colorectal cancer. Obviously, additional validation will be necessary. In vitro functional analyses proved that there is a possibility to regulate the expression level of miRNA in the cells using transfection techniques. However, in future it will be needful to use stable transfection in order to analyze the long term impact of altered expression of miRNA on cell proliferation, apoptosis, cell cycle or metastasing.
