Abstract:

Bakalářská práce pojednává o problematice odpojování pacientů od umělé plicní ventilace (UPV) - weaningu v intenzivní péči. Teoretická část se zabývá činnostmi, které samotnému weaningu předcházejí, a to zajištěním dýchacích cest a UPV. Následuje kapitola zabývající se samotným weaningem, která byla rozebrána v komplexní rovině včetně ošetřovatelské péče v rámci weaningu. Závěrečná kapitola je pouze okrajově věnována teorii adaptačního procesu, slouží jako teoretická opora pro návrh řešení nedostatků, zjištěných v rámci analýzy a interpretace získaných dat. Empirická část práce je zaměřena na úroveň znalostí sester na odděleních intenzivní péče různého charakteru a oddělení ARO Nemocnice ve Frýdku-Místku, p.o. Hlavním výzkumným cílem bylo zjištění úrovně znalostí sester o problematice weaningu. Z analýzy vyplynulo, že 4 oddělení z 5 dosáhla úrovně znalostí vyšší než 70 %. Dále byly zkoumány rozdíly mezi úrovněmi znalostí sester, a to dle délky jejich praxe či oborového zaměření. Dalším předmětem zájmu bylo zjištění, zda respondenti vidí jako přínosnou možnost poskytnutí dokumentu teoretické podpory nově nastupujícím sestrám, který by jim umožnil lepší orientaci v problematice weaningu. V závěru šetření jsme zjišťovali, zda mají respondenti zájem se v této problematice dále vzdělávat.

Abstract:

The bachelor thesis deals with the problems of disconnecting patients from mechanical ventilation - weaning in intensive care. The theoretical part deals with activities that precede weaning itself, namely airway management and mechanical ventilation. The following chapter is dedicated to the weaning itself, which we discuss on a complex level, including nursing care within the framework of weaning. The final chapter is only marginally devoted to the theory of adaptation process, which serves as a theoretical basis for proposing solutions to the shortcomings found in the analysis and interpretation of the data. The empirical part of the work is focused on the level of knowledge of nurses in several intensive care departments and the department of ARO in Frýdek-Místek, p.o. The main research objective was to determine the overall level of nurses' knowledge of weaning. The analysis showed that 4 out of 5 departments had a level of knowledge of more than 70 %. Furthermore, the differences between the nurses' knowledge levels according to the length of their practice or specialization were examined. Another subject of our interest was to find out whether respondents see a document of theoretical support as a beneficial option for new-born nurses, as it would allow them to better understand weaning issues. At the end of the survey we found out whether the respondents are interested in further education in the field of weaning.
