Validation study of Judgment of Line Orientation

Abstract:

Hlavním těžištěm této výzkumné studie bylo ověření psychometrických vlastností Testu orientace čar (JLO), který se používá k diagnostice narušení zrakově-prostorových funkcí. Rozlišovací schopnost testu byla ověřována na souboru 118 respondentů. Kontrolní skupinu tvořilo 91 respondentů bez neurologického, psychiatrického či jiného závažného onemocnění. Klinický soubor byl složen z 27 pacientů s mírnou kognitivní poruchou u Parkinsonovy nemoci (PN-MKP). V této studii nebyl prokázán vliv demografických proměnných, jako byl věk nebo vzdělání, na výkon v testu JLO. Jako statisticky významný se však ukázal být rozdíl ve výkonu v JLO mezi muži a ženami. Muži dosahují vyššího skóre než ženy. Dále diskriminační schopnost Testu orientace čar k odlišení PN-MKP od jeho absence je nedostatečná, výsledná křivka rozlišovací schopnosti testu JLO má náhodnou rozlišovací schopnost. Není proto vhodné užít JLO samostatně jako diagnostický nástroj, který by nám spolehlivě odlišil jedince s PN-MKP od jedinců s přirozeným stárnutím. Jediný signifikantní vztah s testem JLO, i když slabý (ρ = 0,197), byl zjištěn u subtestu Podobnosti z Wechslerovy inteligenční škály pro dospělé (WAIS-R). Vzhledem k tomu, že počet let vzdělání ani věk neměli signifikantní vztah s hodnotami JLO skórů, byly percentilové normy rozděleny dle pohlaví. I když byl pro percentilové normy zvolen malý počet osob, jedná se o první české percentilové předběžné normativní údaje. Lze shrnout, že tato studie prokazuje, že celkový skór testu JLO mezi skupinami PN-MKP a kontrolní skupinou nerozlišuje. Detekční potenciál JLO na individuální úrovni není dostatečný k tomu, abychom mohli tento test používat samostatně bez dalších testů neuropsychologické baterie pro diagnostiku ZP funkcí u PN-MKP nebo dokonce pro diagnostiku celkového kognitivního deficitu u syndromu PN-MKP.

Abstract:

The main focus of this research study is to evaluate on the psychometric properties of the Judgment of Line Orientation Test (JLO). JLO is used to diagnose disruption of visual-spatial functions. The resolving power of the test is validated on the collection of 118 respondents. The control group consisted of 91 respondents without neurological, psychiatric or other serious illness. The clinical group consisted of 27 patients with Mild Cognitive Impairment of Parkinson's disease (PN-MKP). The study does not find evidence of an effect of demographic variables such as age and education on the JLO test performance. However, the difference in performance between women and men in the JLO test has proved to be statistically significant. Men achieve higher score than women. Furthermore, the discriminatory ability of the test to distinguish PN-MKP from its absence is insufficient. The resulting resolution curve of the JLO test has random resolution ability. Therefore, it is not appropriate to use JLO test as a diagnostic tool alone. It would not allow us to reliably distinguish individuals with PN-MKP from the naturally aging individuals. The only significant correlation with the JLO test, even though weak (ρ = 0.197), is found in the Similarities subtest of the Wechsler Intelligence Scale for Adults (WAIS-R). Percentile norms are divided by gender, since the number of education years nor age do not have any significant relationship with the JLO test score values. Although there is small number of people selected for the percentile norms, these are the first percentile preliminary normative data in Czech Republic. To conclude, this study proves that the overall JLO test scores do not differentiate between PN-MKP and control groups. Detection potential of the JLO test on the individual level is not sufficient to enable us to use this test exclusively without any other neuropsychological tests for the diagnosis of ZP functions within individuals with PN-MKP or even for diagnostics of cognitive deficit within PN-MKP syndrome individuals.
