Abstract:

Abstrakt disertační práce Název: Výskyt koaguláza negativních stafylokoků jako původců nozokomiálních nákaz : Fenotypová a molekulárně biologická charakteristika Studie byla zaměřena na charakteristiku grampozitivních koaguláza negativních stafylokoků izolovaných z materiálů nozokomiálních nákaz. Kmeny rodu Staphylococcus získané ze stěrů z nemocničních zařízení byly identifikovány na základě biochemických vlastností do úrovně druhu. Fenotypová charakteristika byla provedena pomocí komerčních setů STAPHYtest 16 a API Staph. Tyto sety byly rozšířeny o řadu dodatkových testů. U kmenů byla sledována tvorba slizu a delta hemolyzinu, což jsou dva důležité faktory virulence. Vyhodnocení identifikace bylo provedeno na PC pomocí programu TNW verze 6. V souboru přibližně 200 kmenů bylo celkem identifikováno 14 druhů a poddruhů koaguláza negativních stafylokoků. Nejčastěji byl zastoupen druh Staphylococcus epidermidis (45,1 %), Staphylococcus hominis subsp. hominis (14,4 %) a Staphylococcus. haemolyticus (10,2 %). Testovaný soubor byl hodnocen na základě úspěšnosti identifikace. Ze sledovaného souboru byly vybrány pro bližší typizaci kmeny Staphylococcus epidermidis, u kterých byla provedena kromě biotypizace i ribotypizace. Biotypizace Staphylococcus epidermidis neprokázala významnou vnitrodruhovou variabilitu těchto izolátů, atypické reakce se, až na tři kmeny, nevyskytovaly. Naproti tomu výsledky ribotypizace ukázaly větší heterogenitu kmenů a jednoznačně prokázaly souvislost mezi ribotypem a místem odběru vzorků. Nalezené ribotypy se navíc v prostředí opakují v delším časovém rozmezí, což svědčí o výskytu a přetrvávání stejných kmenů podmíněně patogenních bakterií v nemocničním prostředí. Získané výsledky naznačují, že ribotypizace je vhodnou metodou umožňující precizní a spolehlivou detekci některých koaguláza negativních stafylokoků, která má význam také pro epidemiologickou analýzu infekcí vyvolaných těmito kmeny.

Dissertation Abstract Title: Occurrence of coagulase negative staphylococci as a source of hospital infection: Phenotypic and molecular characterization The study was investigated on the characteristic of grampositive coagulase negative staphylococci isolated from the hospital materials. The Staphylococcus strains acquired from scrapings from hospital environments were identified to the species level based on their biochemical properties. Phenotypic characteristic were performed by commercial identification kit STAPHYtest 16 and API Staph. These sets were supplemented with several additional tests. The production of slime and delta haemolysin was tested in the strains as the important virulence factors. The evaluation of identification was performed by programme TNW version 6. In the set of approximately 200 strains there were identified 14 different species and subspecies of coagulase negative staphylococci. Among them the most frequent species which were found out, belonged to Staphylococcus epidermidis (45,1 %), Staphylococcus hominis subsp. hominis (14,4 %) and Staphylococcus haemolyticus (10,2 %). Tested set was evaluated on the basis of success of the identification. From the monitored sample the Staphylococcus epidermidis strains were selected for more accurate typing and tested on their virulence factors, biotyping and ribotyping. The biotyping of S. epidermidis did not show any considerable intraspecific variation of these isolates and there were no atypical reactions, with the exception of three strains. On the contrary, the results of ribotyping showed greater heterogeneity of strains and unequivocally demonstrated the relation between the ribotype and the place of sample drawing. In addition to this fact, found ribotypes repeat in the same environment in the long period, which is the evidence of occurrence and persistence of the same strains of conditionally pathogenic bacteria in the hospital environment. The acquired results indicate that the ribotyping is a suitable method enabling precise and reliable detection of some coagulase negative staphylococci. This fact is also very important for epidemiological analyses of infections induced by these strains.
