Overtraining syndrome - a systematic overview

Abstract:

Cílem této práce bylo shrnutí současných vědeckých poznatků na téma syndrom přetrénování. Práce se zaměřuje na diagnostické metody, prevenci a léčbu. K vyhledávání článků byly využiti databáze Web of science, PubMed, a Medline. Celkem bylo do přehledu zahrnuto sedm studií. Šest studií se zabývalo diagnostickými metodami syndromu přetrénování, a jedna studie se zabývala prevencí a léčbou. Diagnostické metody syndromu přetrénování jsou různorodé. Ve studii od Cadegiani & Kater (2017) navrhli mezní hodnoty kortizolu 30 minut po probuzení, kortizolu a adrenokortikotropního hormonu (ACTH), a zvýšení kortizolu a ACTH během inzulinového tolerančního testu pro diagnostiku syndromu přetrénování. Cadegiani & Kater (2018) se dále zabývali diagnostickou metodou pomocí růstového hormonu a prolaktinu, kde také navrhli mezní hodnoty těchto ukazatelů. Studie Cadegiani et al. (2020) se zabývala kombinacemi klinických a biochemických ukazatelů, kombinace více ukazatelů, by mohly být pro diagnostiku syndromu přetrénování nejpřesnější metodou. Nicoll et al. (2018) testovali koncentraci hormonů štítné žlázy, ovšem zjistili že z praktického hlediska není tento ukazatel vhodným diagnostickým nástrojem. Ushiki et al. (2020) zjistil, že automatizovaným měřením kortizolu pomocí elektrochemiluminiscenční imunoanalýzy (ECLIA) je možné detekovat cirkadiánní rytmus sportovců a porovnat intenzitu zátěže při cvičení ve stejnou dobu v průběhu různých dnů, a dokonce i v brzkých ranních hodinách. Diagnostickou metodu se pokusili navrhnout i výzkumnici Kiviniemi et al. (2014) předmětem výzkumu bylo zhodnotit individuální vztah mezi R-R intervalem (RRi) a vágově zrpostředkovanou variabilitou RRi, výzkum zjistil že toto zhodcocení může být zásadní informací pro včasnou diagnostiku syndromu přetrénování a zotavení se z něj u vytrvalostních sportovců. Poslední nalezená studie popisuje prevenci a léčbu, jednalo se o konsensuální prohlášení a obecné doporučení pro trenéry a lékaře od Meeusen et al. (2013). Diagnostika syndromu přetrénování je obtížná a přesto, že byly výsledky popisovaných studií pozitivní, žádná z nich nebyla definitivní, a je potřeba provést další studie k potvrzení. Důležité jsou preventivní opatření jako pasivní volno 1 den v týdnu, individualizace tréninků, spánek, výživa, screening psychického stavu sportovce. Léčebné opatření se prolíná s preventivními doporučeními.

Abstract:

The aim of this bachelor thesis was to summarize current scientific knowledge on the topic of burnout syndrome. Thesis focused on diagnostic methods, prevention and treatment. Databases Web of science, PubMed and Medline were used for searching for articles. In total, seven studies were included in the research. Six studies dealt with diagnostic methods of overtraining syndrome, and one study dealt with prevention and treatment. Diagnostic methods of overtraining syndrome are diverse. In a study by Cadegiani & Kater (2017), they suggested limit values for cortisol 30 minutes after awakening, cortisol and adrenocorticotropic hormone (ACTH), and increase in cortisol and ACTH during an insulin tolerance test for the diagnosis of overtraining syndrome. Cadegiani & Kater (2018) further dealt with the diagnostic method using growth hormone and prolactin, where they also suggested the limit values of these indicators. A study by Cadegiani et al. (2020) dealt with combinations of clinical and biochemical indicators. Combinations of multiple indicators could be the most precise method for diagnosing overtraining syndrome. Nicoll et al. (2018) tested the concentration of thyroid hormones, but found that from a practical point of view, this indicator is not a suitable diagnostic tool. Ushiki et al. (2020) discovered, that by automated measurement of cortisol using electrochemiluminescence immunoassay (ECLIA) it is possible to detect the circadian rhythm of athletes and to compare the intensity of the exercise load at the same time during different days, and even in the early hours of the morning. Researchers Kiviniemi et al. (2014) also tried to design a diagnostic method. The subject of the research was evaluation of the individual relationship between R-R interval (RRi) and vaguely centered variability of RRi. The research found that this evaluation can be essential information for early diagnosis of overtraining syndrome and recovery from it for endurance athletes. The last study found, which described prevention and treatment, was a consensus statement and general recommendations for trainers and clinicians by Meeusen et al. (2013). The diagnosis of overtraining syndrome is difficult and although the results of the described studies were positive, none of them were definitive, and further studies are needed to confirm. Preventive precautions are also important, such as passive rest 1 day a week, individualization of training, sleep, nutrition and screening of the mental state of the athlete. Treatment precautions are combined with preventive recommendations.
