Network analysis of healthcare avaiability in GIS

Abstract:

Dostupnost zdravotní péče je klíčová pro správný chod systému zdra-votnictví. Jednou z rovin, jak na ni nahlížet je rovina geografická, tzv. místní dostupnost zdravotní péče, která je v ČR upravena nařízením vlády č. 307/2012 Sb. Toto nařízení specifikuje maximální dojezdové doby k místu poskytování zdravotní péče, pro primární péči činí tato doba 35 minut. V této práci se věnuji analýze místní dostupnosti zdravotní péče kombinací software TravelTime a geografického informačního systé-mu QGIS, a modelem věkové struktury lékařů od současných dat po dobu 20 let dopředu, založeným na Leslieho populačním modelu. Vstupními daty pro analýzu geografické dostupnosti byly údaje o ordi-nacích praktických lékařů a praktických lékařů pro děti a dorost, které pocházejí z Národního registru poskytovatelů zdravotních služeb, hod-nocení věkové struktury proběhlo na kategorizovaných datech o věku na výše zmíněném výběru lékařů, a to na úrovni okresů. Tato data po-cházela z Národního registru zdravotnických pracovníků. Vizualizovány byly výsledky pro Ústecký kraj; v rámci dostupnosti nebylo shledáno žádných zásadních změn. Predikce věkové struktury byla znázorněna pro všechny okresy ČR; vyplývá z ní, že počet lékařů ve věku 65 a více let bude v následujících 20 letech stoupat.

Abstract:

Access to care is crucial for functioning of a healthcare system. One way to measure access to care is its geographical aspect, often called the spatial accessibility of healthcare. In the Czech Republic, the maximum time of a patient travelling to reach primary care by car should not ex-ceed 35 minutes, as set by government decree no. 307/2012. In this thesis, we analyse the accessibility of healthcare with the use of software TravelTime, geographical information system QGIS and our own model of age structure of general practitioners and general practitioners for children. This model is based on Leslie population model and we use it to predict development of the age structure during the next 20 years, based on present day data. The input data for our analysis of accessibility are the contempo-rary doctor’s offices, which were retrieved from the National registry of healthcare providers. The input data for prediction of age structure of general practitioners and general practitioners for children was re-quired from the National registry of healthcare workers, this data was aggregated by districts and grouped into age categories. Our visualisation of the outcomes of the accessibility analysis for the Ústecký region do not show significant change in geographical ac-cessibility of healthcare. However, by applying our prediction model to the data from the whole of Czech Republic, we observe that the number of general practitioners aged 65 or more is going to rise in the next 20 years, contributing evidence for a necessary action by policymakers to avoid accessibility problems on a longer-term horizon.
