Medical social worker in the process of a patients' dismissal from the hospital

Abstract:

Bakalářská práce se zabývá tématem zdravotně sociálního pracovníka v procesu propouštění pacientů z nemocnice. Cílem je zjistit, jak se zdravotně sociální pracovník zapojuje v procesu propouštění pacientů z nemocnice. Práce se skládá z části teoretické a aplikační. V teoretické části je popsána historie práce zdravotně sociálního pracovníka, struktura nemocnice, popis procesu propouštění z nemocnice a následné péče, se kterými se zdravotně sociální pracovníci spolupracují. Cílem aplikační části je zjistit, jaké jsou překážky při zajišťování následné péče v procesu propouštění pacienta z nemocnice, se kterými se zdravotně sociální pracovníci setkávají. Aplikační část je zpracována formou návrhu kvantitativního výzkumu se sběrem dat prostřednictvím techniky dotazníku.

Abstract:

The bachelor's thesis deals with the topic of a medical social worker in the process of discharging patients from a hospital. The aim is to find out how the medical social worker is involved the process of patients' dismissal from the hospital. The work consists of theoretical and practical parts. The theoretical part describes the history of the work of a medical social worker, the structure of the hospital, description of the process of discharge from the hospital and aftercare, with which medical social workers work. The aim of the practical part is to find out what the obstacles are in providing follow-up care in the process of discharging a patient from the hospital, with which medical social workers meet. The practical part is processed in form a concept in quantitative research with data collection through a questionnaire technique.
