Constraint Induced Movement Therapy in stroke patients

Abstract:

Tato diplomová práce se zabývá hodnocením účinnosti léčby akutních cévních mozkových příhod pomocí kombinované terapie (standardní rehabilitace a terapie formou vynuceného používání) v klinické praxi. Léčbu úspěšně dokončilo celkem 7 pacientů hospitalizovaných na I. Neurologické klinice Fakultní nemocnice U sv. Anny v Brně. Účinnost léčby jsme ověřovali pomocí těchto testů: Chedoke - McMaster Hemiplegia Assessment, Barthel Index, Functional Independence Measure a Ashwortova modifikovaná škála. Při využití terapie formou vynuceného používání v kombinaci se standardní rehabilitační léčbou došlo u všech sledovaných parametrů ke statisticky významnému zlepšení: Chedoke - McMaster Hemiplegia Assessment (vstup 22,1 ± 1,4 bodů; výstup 24,7 ± 0,5 bodů; p = 0,017), Barthel Index (vstup 77,9 ± 16,0 bodů; výstup 95,7 ± 8,6 bodů; p = 0,017), Functional Independence Measure (vstup 103,1 ± 20,7 bodů; výstup 121,9 ± 6,6 bodů; p = 0,017). Při porovnání míry zlepšení funkční soběstačnosti sledovaného souboru (n= 7; průměrný věk 59,4 ± 3,9 let; průměrná délka hospitalizace 10,5 ± 4,1 dní; standardní rehabilitace 1 x 30 minut denně a terapie formou vynuceného používání 1 x 30 minut denně), podstupujícího kombinovanou terapii, s kontrolním souborem (n= 7; průměrný věk 62,1 ± 4,1 let; průměrná délka hospitalizace 11,1 ± 3 dní; standardní rehabilitace 1 x 30 minut denně), podstupujícím pouze standardní rehabilitační léčbu, ale nebyl zjištěn statisticky významný rozdíl mezi oběma soubory (Barthel Index: p = 0,65; Functional Independence Measure: p = 0,74).

Abstract:

This diploma thesis deals with evaluation of the effectiveness of acute stroke treatment using the combination therapy (standard rehabilitation therapy and Constraint Induced Movement Therapy) in clinical practice. A total of 7 patients hospitalized at the Department of Neurology, St. Anne‘s University Hospital in Brno, successfully underwent the treatment. We tested the effectiveness of the treatment using the following tests: Chedoke - McMaster Hemiplegia Assessment, Barthel Index, Functional Independence Measure and Ashworth Modified Scale. The combination of the Constraint Induced Movement Therapy and the standard rehabilitation therapy showed significant statistical improvement in all parameters monitored in patients: Chedoke - McMaster Hemiplegia Assessment (input 22.1 ± 1.4 points; output 24.7 ± 0.5 poitnts; p = 0.017), Barthel Index (input 77.9 ± 16.0 points; output 95.7 ± 8.6 points; p = 0.017), Functional Independence Measure (input 103.1 ± 20.7 points; output 121.9 ± 6.6 points; p = 0.017). We found no statistically significant difference between the treated group (n= 7; average age 59.4 ± 3.9 years; average length of hospitalization 10.5 ± 4.1 days; standard rehabilitation therapy 1 x 30 minutes per day and Constraint Induced Movement Therapy 1 x 30 minutes per day) and the control group (n= 7; average age 62.1 ± 4.1 years; average length of hospitalization 11.1 ± 3 days; standard rehabilitation therapy 1 x 30 minutes per day) in the level of functional independence (Barthel Index: p = 0.65; Functional Independence Measure: p = 0.74).
