The health status of foreigners living in Ostrava

Abstract:

Zdravotní stav cizinců žijících v Ostravě Úvod: V České republice žije více než 400 tisíc cizinců. V Moravskoslezském kraji je to více než 23 tisíc a v okrese Ostrava je to asi 10 tisíc cizinců, z nichž většina žije v samotné Ostravě. Česká republika je zemí, kde se počet cizinců od 90. let zvyšuje téměř každým rokem. Informací o zdravotním stavu cizinců je však nedostatek a nezahrnují všechny skupiny cizinců. Cílem praktické části práce je analýza dat z dotazníkového šetření, které bylo cíleno na dospělé pacienty ? cizince, kteří jsou registrováni u praktických lékařů pro dospělé na území Ostravy. Metodika: Data o registrovaných cizincích byla získána pomocí dotazníků v ordinacích praktického lékaře. Do šetření se zapojilo 51 lékařů registrovaných na území Ostravy. Dotazníkem byly zjišťovány demografické údaje, údaje o zdravotním stavu, schopnosti komunikovat a přístup k preventivním prohlídkám. Pro vyhodnocení dat byla použita základní popisná statistika, chí-kvadrát test a logistická regrese - výpočet hrubého OR (Odds Ratio) a adjustovaného OR s 95% intervaly spolehlivosti (95% IS). Statistické testy byly hodnoceny na hladině významnosti 5 %. Zpracování bylo provedeno v programu Stata verze 13. Výsledky: Zkoumaný soubor tvoří 386 pacientů. Nejvíce pacientů pochází z Vietnamu (50 %), 21 % z Polska a Slovenka. Muži byli zastoupeni z 60 %, 55 % souboru tvořily osoby ve věku 36-55 let. Základní vzdělání má 31 % cizinců, středoškolské 29 %, vysokoškolské 13 % a u 27 % cizinců nebylo vzdělání zjištěno. Dle záznamů z lékařské dokumentaci bylo v souboru 17 % kuřáků. Obezitou (BMI ? 30 kg/m2) trpělo 10 %. Celkově hodnotili lékaři u 59 % zdravotní stav jako dobrý (stav velmi dobrý nebo přiměřený věku) a u 41 % osob jako špatný (trpí nějakým druhem chronického onemocnění nebo jiným závažným onemocněním). Ze sledovaných faktorů se na zdravotním stavu negativně projevuje etnikum, věk, BMI a kouření. Ve srovnání se skupinou cizinců ? Poláků a Sloválů vykazují vietnamští státní příslušníci horší zdravotní stav (OR = 2,62, 95% IS: 1,3-5,28). Závěr: Zdravotní stav cizinců byl označen jako špatný (trpí nějakým druhem chronického onemocnění nebo jiným závažným onemocněním) u 41 %. Ze sledovaných skupin ciziniců mají nejhorší zdravotní stav vietnamští příslušníci.

Abstract:

Foreign Citizens´ State of Health in Ostrava City Introduction: In the Czech Republic there are more than 400 thousand foreigners. In the Moravia-Silesian region there are more than 23 thousand, in Ostrava district about 10 thousand, where most of them live in the city of Ostrava. The Czech Republic belongs to the countries where the number of foreigners has been increasing almost every year since the 90s. There is the lack of information concerning foreigners´ state of health as well as not all groups of foreigners are mentioned in the information. The aim of the practical part of this dissertation is the analysis of data from the survey, which was targeted at adult foreigners who are registered by general practitioners in Ostrava. Methods: Data about registered foreigners were obtained through questionnaires from genaral practioners´ offices. The survey involved 51 doctors registered in Ostrava. Questionnaires were collected and categorized according to demographic data, health data, ability to communicate and approach to preventive examinations. Basic descriptive statistics, chi-square test and logistic regression was used to analyze the data - calculation of gross OR (Odds Ratio) and the adjusted OR with 95% confidence intervals (95% CI). Statistical tests were evaluated at significance level of 5%. Processing was performed in program Stata, version 13. Results: The research sample consists of 386 patients. The most of patients come from Vietnam (50%), 21% from Poland and Slovakia. Men were represented by 60%, 55% of the sample consisted of persons aged 36-55 years. 31% of foreigners has basic education, 29% comes from high schools, 13% has college education and no information was found about their education for 27% of foreigners. According to the medical records there were 17% of smokers in the evaluated sample and 10% suffered from obesity (BMI ? 30 kg / m2). Overall, physicians rated a good state of health( adequate to the age) in 59% and a poor one (suffering from some kind of chronic illness or other serious diseases) in 41%. Factors which affected negatively the state of health in this analysis are ethnicity, age, BMI and smoking. When comparing with the group of Polish and Slovak foreigners the Vietnamese ones shows a worse state of health (OR = 2.62, 95% CI: 1.3 to 5.28). Conclusion: The foreigners´ state of health was identified as poor based on suffering from some kind of chronic illness or other serious diseases in 41% of analyzed sample. The worst state of health in the observed group belongs to Vietnamese foreigners.
