The nursing process of newborn with controlled hypothermia

Abstract:

Cílem této bakalářské práce s názvem Ošetřovatelský proces v péči o novorozence s řízenou hypotermií je vypracování názorného plánu ošetřovatelské péče vycházejícího z ošetřovatelských diagnóz NANDA International Taxonomie II, které se vztahují k potřebám hospitalizovaného novorozence s diagnózou hypoxicko-ischemická encefalopatie léčeného řízenou hypotermií. Předložená bakalářská práce obsahuje teoretickou a praktickou část. Teoretická část je rozdělena na čtyři kapitoly. První kapitola popisuje postnatální adaptaci dítěte. Ve druhé kapitole je popsána perinatální asfyxie, hypoxicko-ischemická encefalopatie a její jednotlivé fáze, resuscitace novorozence. Ve třetí kapitole je popsána léčba aktivní řízenou hypotermií. Čtvrtá kapitola je zaměřena na ošetřovatelskou péči o dítě před, během a po léčbě řízenou hypotermií. Je zde popsáno přijetí dítěte na jednotku intenzivní a resuscitační péče pro novorozence (JIRPN), zavádění invazivních vstupů, zajištění dítěte a odběry biologického materiálu. V neposlední řadě je popsána také psychická péče o rodinu dítěte. V praktické části byl vytvořen plán ošetřovatelské péče vycházející z ošetřovatelských diagnóz NANDA International Taxonomie II pro konkrétního novorozence s hypoxicko-ischemickou encefalopatií indikovaného k léčbě aktivní řízenou hypotermií. Byly stanoveny také ošetřovatelské diagnózy související s potřebami rodičů dle filozofie péče orientované na rodinu. Po dobu hospitalizace děvčátka na JIRPN bylo stanoveno 9 aktuálních a 3 potencionální ošetřovatelské diagnózy dle NANDA International Taxonomie II. Na základě stanovených ošetřovatelských diagnóz byly vymezeny cíle ošetřovatelské péče, popsány ošetřovatelské intervence, realizace ošetřovatelské péče a následně byly cíle vyhodnoceny.

Abstract:

The objective of this bachelor's thesis titled "The Nursing Process in Care of Newborn with Therapeutic Hypothermia" is an elaboration of an illustrative nursing plan derived from the nursing diagnoses NANDA International Taxonomy II. The purpose of the plan applies to the needs of a hospitalized newborn with a diagnoses of hypoxic-ischemic encefalopathy treated with therapeutic hypothermia. This bachelor's thesis consists of two parts: a theoretical and practical part. The theoretical section is divided into four chapters. The first chapter describes newborn's adaptation to the extrauterine life during the immediate postnatal period. The second chapter defines the perinatal asphyxia, etiology and diagnostics. First subchapter focuses on what hypoxic-ischemic encefalopathy is and its stages. Second subchapter describes neonatal resuscitation. Third chapter explains therapeutic hypothermia. Fourth chapter focuses on the nursing care of newborn before, during and after treatment with therapeutic hypothermia. The admission process of newborn into the neonatal intensive care unit (NICU), obtaining invasive access and sampling of biological material are described. Lastly, the attention is focused on psychological care and support of the newborn's family. In the practical part, the plan of a nursing care is created in reference to the nursing diagnosis NANDA International Taxonomy II for the particular case of a newborn with hypoxic-ischemic encefalopathy receiving the therapeutic hypothermia treatment. There was also diagnosis related to the needs of parents according to the family-centered philosophy. During the female newborn's hospitalization on NICU, there were 9 actual and 3 potentional nursing diagnoses established according to the NANDA International Taxonomy II. Based on the stated nursing diagnoses, goals were defined for appropriate nursing care. The goals were derived from described nursing interventions and implementation of the nursing care, and the goals were subsequently evaluated using the aforementioned framework.
