Problem and occurence of enamel demineralization in patients with fixed orthodontic appliances.

Abstract:

Tato práce se skládá ze dvou základních částí. První z nich je část teoretická, která má za cíl shrnout současné poznatky o problematice demineralizací skloviny vzniklých u pacientů léčených fixním ortodontickým aparátem. Pro lepší porozumění celé problematiky je zde nejprve popsána makroskopická i mikroskopická stavba skloviny a její funkce. Posléze práce popisuje základní charakteristiku demineralizací, jejich etiologii, histologickou podobu, diferenciální diagnostiku a výskyt. Cíl předkládané práce má preventivní charakter, proto je na kapitolu věnované prevenci vzniku a rozvoje demineralizací kladen značný důraz. Rozebírá v současné době používaná preventivní opatření, které vykonává jak pacient sám, tak ortodontista či dentální hygienistka. V závěru teoretické části je popsána léčba demineralizací od metod nejjednodušších po metody složitější a invazivnější. Druhá, praktická část této práce se zabývá nejčastější lokalizací výskytu demineralizací u sledované skupiny pacientů po léčbě fixním ortodontickým aparátem a jeho statistickým zpracováním. Zabývá se tedy tím, jaké druhy zubů a části jejich vestibulárních ploch jsou demineralizacemi nejčastěji postiženy, zda je rozdíl ve výskytu mezi horním a dolním zubním obloukem, taktéž mezi zuby frontálního a distálního úseku chrupu a také zda je rozdíl mezi pravou a levou polovinou chrupu. Poznatky z praktické i teoretické části této práce byly podkladem pro zhotovení informačního letáku, který je součástí praktické části a je vhodný jak pro samotné pacienty či jejich rodiče, tak pro ortodontisty a dentální hygienistky. Cílem zhotovení tohoto letáku bylo zvýšení informovanosti o problematice demineralizací, souvisejících s ortodontickou léčbou a jeho využití jako motivační pomůcky.

Abstract:

The thesis consists of two main parts. The first is a theoretical part, which aims to summarize the current knowledge about the problems of enamel demineralization resulting in patients treated with fixed orthodontic appliances. For a better understanding of the whole issue there is first discussed macroscopic and microscopic structure of enamel and its functions. Eventually it describes the basic characteristics of demineralization, their etiology, histological appearance, differential diagnosis and incidence. Aim of the thesis is preventive in nature, so there is a considerable emphasis on a chapter devoted to demineralization prevention and development. There is examined currently precaution that is performed by a patient himself and by an orthodontist or a dental hygienist. In the end of the theoretical part there is described treatment of demineralization from the simplest to the most complex and invasive methods. The second part of the thesis deals with the most common sites of demineralization occurrence of the monitored group of patients after treatment with fixed orthodontic appliance and its statistical processing. It explores what kinds of teeth and parts of their vestibular demineralization areas are most affected, whether the difference in occurrence between the upper and lower dental arch, as well as between frontal and distal part of the dentition and also whether there is a difference between the right and left half of dentition. Knowledge of practical and theoretical part of the thesis was the basis for the leaflet, which is a part of the practical part and it is suitable both for the patients themselves or their parents, orthodontists and dental hygienists. The aim of making this leaflet was to increase awareness about demineralization associated with orthodontic treatment and its use as motivational tools.
