Nursing Care of a Patient with respiratory Failure on ECMO and Ventilator Focusing on Nursing Standards Methodology

Abstract:

Mikolášková, Zuzana. Ošetřovatelský proces u pacienta na UPV a ECMO se zaměřením na tvorbu ošetřovatelských standardů. Vysoká škola zdravotnická, o. p. s. Stupeň kvalifikace: Bakalář (Bc.). Vedoucí práce: Mgr. Ondřej Ulrych, PhD. Praha 2023. 100 s. Pandemie covid 19 ukázala, že možnosti managementu péče u pacientů s respiračním selháním jsou daleko rozsáhlejší. Kromě standardních postupů byly získány zkušenosti, jak v daleko větší míře používat, kromě umělé plicní ventilace, i terapii vysoko průtokovým kyslíkem a mimotělní membránovou oxygenaci. Během pandemie se použití metody ECMO rozšířilo a počet pacientů se zvýšil. To byl jeden z důvodů, proč je tématem práce péče o pacienty s ECMO a přibližuje tak náročnou péči i dalším zdravotníkům. Dalším důvodem byl i složitý průběh hospitalizace popisovaného pacienta a jeho úspěšná léčba. Respirační selhání je život ohrožující stav, při kterém respirační systém není schopen zajistit adekvátní výměnu plynů. Výsledkem je hyperkapnie nebo hypoxémie nebo obojí současně. Aby bylo možné o takto náročné pacienty pečovat, je důležité, aby sestry znaly postupy a ošetřovatelské standardy týkající se ECMO (BERSTEN, HANDY, 2019). Cílem této práce je popsat druhy respiračního selhání, možnosti léčby pomocí UPV, high flow, ECMO a využití polohování náklonem a pronační polohy. Další částí je vypracování ošetřovatelského plánu pro konkrétního pacienta dle modelu Marjory Gordonové. Přínosem této práce je vytvoření návrhu ošetřovatelského standardu u pacientů s respiračním selháním na UPV a ECMO.

Ondřej Ulrych, PhD. Prague 2023. The covid 19 pandemic has shown that the care management options for patients with respiratory failure are far more extensive. In addition to standard procedures, experience has been gained in the far greater use of high-flow oxygen therapy and extracorporeal membrane oxygenation in addition to artificial pulmonary ventilation. During the pandemic, the use of ECMO became more widespread and the number of patients increased. This was one of the reasons why the care of ECMO patients is the topic of the paper, bringing the challenging care to other health professionals. Another reason was the complicated course of the hospitalization of the described patient and his successful treatment. Respiratory failure is a life-threatening condition in which the respiratory system is unable to provide adequate gas exchange. The result is hypercapnia or hypoxaemia or both simultaneously. In order to care for such challenging patients, it is important to, that nurses are familiar with the procedures and nursing standards related to ECMO (BERSTEN, HANDY, 2019). Aim of this thesis is to describe the types of respiratory failure, treatment options using mechanical ventilation, high flow, ECMO and the use of prone positioning. The next part is to develop a nursing plan for a specific patient according to the Marjory Gordon model. The contribution of this work is the development of a draft nursing standard for patients with respiratory failure on artificial mechanical ventilation and ECMO.
