Malignant melanoma of the uvea

Abstract:

Malígny melanóm uvey (MMU) je najčastejší primárny vnútroočný nádor u dospelých. Cieľom práce bolo 1. podľa klinických a histologických parametrov (vek, pohlavie, lokalizácia tumoru, príznaky tumoru a ich dĺžka trvania, spôsob chirurgickej liečby, histologický typ tumoru, prítomnosť vzdialených metastáz, TNM klasifikácia) spracovať súbor všetkých pacientov, ktorí boli v rokoch 2000 až 2006 na Očnej klinike FN Brno chirurgicky liečený pre MMU, 2. imunohistochemicky vyšetriť expresiu tumor-supresorového génu p53 v bunkách malígneho melanómu uvey z formol-parafínového materiálu, 3. výsledky imunohistochemickej detekcie korelovať s klinickými znakmi tumoru u jednotlivých pacientov. Metodika: Súbor tvorilo 37 pacientov (19 žien a 18 mužov) priemerného veku 63,95. Pacienti boli klinicky kompletne vyšetrený, laboratórne bola vyšetrená imunohistochemická detekcia expresie génu p53 a p21. Výsledky: MM chorioidey bol diagnostikovaný u pacientov, MM riasnatého telesa u pacientov, MM iris u pacientov. MMU bol u 5 pacientov asymptomatický, u ostatných sa prejavil poklesom zrakovej ostrosti alebo bolesťou. Metastázy boli nájdené u 7 pacientov. Abnormálna expresia génu p53 bola detekovaná u 6 z 37 pacientov, génu p21 u 4 z 29 pacientov. (intenzita imunostainingu 1-5%). Záver: Napriek súčasným diagnostickým možnostiam je malígny melanóm uvey často diagnostikovaný v neskorom štádiu, ktoré vyžaduje radikálne chirurgické riešenie bez možnosti zachovania užitočných zrakových funkcií. Výsledky imunohistochemickej detekcie expresie génu p53 a p21 nekorelujú s lokalizáciou tumoru, histologickým typom a metastázami.

Abstract:

Malignant melanoma of the uvea (MMU) is the most common intraocular tumor of adults. Purpose: 1. to evaluate various criteria of MMU (age, sex, localization of MMU, clinical signs of MMU, size of MMU, methods used in diagnostic, treatment, histological type, TNM classification and metastases) in a group of all patients, to whom was in years 2000-2006 diagnosed and surgical treated MMU, 2. to evaluate immunochemical expression of tumor-suppressor gene p53, 3. to correlate localization, histological type and metastases with results of immunochemical detection. Methods: Between years 2000 to 2006 had been in the Department of Ophthalmology diagnosed and surgical treated MMU in 37 patients (19 women and 18 men) with average age 63,95. 37 patients were clinically completely examined, paraffin-embedded specimen were immunochemically analyzed for protein expression of p53 and p21. Results: MM of choroid was diagnosed in 8 cases, MM of ciliary body in 2 cases and MM of iris in 1 patient. MMU was in 5 patients asymptomatic, in all the other cases was manifested with the decrease of visual acuity or pain. Metastases had been diagnosed in 7 patients. p53 staining was positive in 6 of 37 patients, ranging between 1-5%. p21 staining was positive in 4 of 29 patients between 1-5%. Conclusion: Despite of currently diagnostic possibilities is the majority of MMU diagnosed in late stage, that requires radical therapeutic solution. Knowledge in variety of clinical performance of MMU can help to early catchment of MMU, that will afford opportunity of therapy, which particulary spare visual functions. Results of immunochemically detection statistically do not correlate with histological type, localization and metastases.
