The Link between Significant Events and Therapeutic Outcome - A Case Study of Therapy of a Client with Depressive Symptoms

Abstract:

Tato diplomová práce zkoumá terapeutickou změnu v případu střednědobé terapie klientky s depresivní symptomatikou z pohledu paradigmatu významných momentů. Úvodní část je věnována zejména přehledu dosavadních výzkumů s podobnou tematikou, kterých je poměrně malé množství a mají velký rozptyl výzkumného zájmu. Práce si proto kladla za cíl rozšířit poznatky výzkumu o roli významných momentů explorativní analýzou spojitosti mezi momenty a výsledky terapie, jenž je inspirována dřívějším výzkumem Timuľáka et al. (2010). Z analýzy vzešlo 7 kategorií momentů na základě jejich nalezené spojitosti se změnou. V momentech byly nalezeny terapeutické faktory, které jsou mnohými autory považovány za společné faktory změny. Ukázali jsme, že propojení s celkovou změnou je vysledovatelné v každém jednotlivém momentu a že terapeutický přínos některých momentů by mohl spočívat v tom, že vytvářejí podmínky pro vznik jiných terapeuticky významných momentů. Jde zejména o momenty primárně podporující terapeutický vztah, kterých je v našem výzkumu přibližně třetina. V našich výsledcích jsme našli podobnosti s výsledky jiných výzkumů terapeutické změny. Ukázali jsme také, že bezprostřední účinek momentu na klienta je úzce spjat s celkovou změnou. Navrhli jsme výzkumy do budoucna, které by dále obohatily chápání role významných momentů v terapeutickém procesu a překonaly by některé limity naší studie.

Abstract:

The present thesis examines therapeutic outcome in a case of medium-term therapy of a client with depressive symptoms, from the perspective of significant event paradigm. In the Introduction part, it deals with an overview of existing research on this or similar topic, which is just a small group of studies with a broad range of focus. Our study aimed to extend the existing knowledge about the role of significant events. It applied exploratory analysis of the link between these events and the outcome inspired by previous study of Timulak et al. (2010). We identified 7 categories of events based on their link to the outcome. Therapeutic factors that were found within those events are considered, by many authors, as common factors of change. We demonstrated that the link is trackable in every single event and the main contribution to the outcome in some events might be that they create conditions for origination of other therapeutic events. These are especially events that promote relational aspect of the therapy. Approximately one third of significant events in our study belong to this case. Our results are in many ways similar to previous research findings on therapeutic change. We also discovered that immediate impacts of the events are closely connected with overall outcome. We suggested possible future research that would enrich present understanding of the role of significant events in the process of therapy and would overcome some limits of our study.
