Outpatient delivery yes or no?

Abstract:

Cíl: Cílem diplomové práce je zjistit postoje a názory zdravotnických pracovníků z odlišných nemocnic na problematiku ambulantního porodu v České republice. Součástí je porovnání těchto názorů ze dvou hledisek ? pediatrického a gynekologického, posouzení negativních a pozitivních stránek alternativní metody porodu. Metodika: Data k výzkumnému šetření byla získána vyplněním anonymního dotazníku. Dotazník obsahoval celkem 17 otázek. Zkoumaný soubor byl sestaven z 225 zdravotnických pracovníků. Sběr dat probíhal od srpna 2012 do listopadu 2012. Pro statistické vyhodnocení byl aplikován ?2 test. Při nedostatečném počtu respondentů bylo zapotřebí Fischerova exaktního testu. Statistické testy byly vyhodnoceny na hladině významnosti 5 %. Výsledky: V naší práci bylo zjištěno, že 22 % respondentů přesně zná, co ambulantní porod znamená, 66 % tuší a 12 % dotazovaných neví. 70 % zdravotnického personálu ambulantní porod nepodporuje a 39 % respondentů se s ním v praxi ještě nikdy nesetkalo. Napříč tomu si 64 % respondentů myslí, že ženy, které ambulantní porod podstupují, jsou informované o možných komplikacích, jež mohou nastat. Jako nejčastější zdroj informací uvádějí zdravotníci v 34 % internet. Pro 45 % dotazovaných je nejčastějším důvodem podstoupení ambulantního porodu stres v nemocničním prostředí. Závěr: Většina dotazovaných zdravotnických pracovníků ambulantní porod nepodporuje. Důvody, které uváděli pro i proti ambulantnímu porodu se z hlediska pediatrického vyjadřovaly spíše ke komplikacím a problematice novorozence, zatímco z pohledu gynekologického řešily problematiku ženy po porodu. Všechny profesní skupiny ambulantní porod upřednostňují před porodem doma.

Abstract:

Aim: The aim of this thesis is to investigate the attitudes and opinions of the employees in the healthcare on outpatients delivery issues in different hospitals in the Czech Republic. Part of the aims is the comparison of the opinions mentioned above from two points of views ? the pediatric one and the gynecological one, including the assessment of negative and positive aspects of alternative methods of child delivery. Methods: Data have been obtained from anonymous questionnaires. The questionnaire included a total of 17 questions. Two hundred twenty five medical professionals took part in this survey. The data were collected from August 2012 until November 2012 and ?2 was applied for statistical evaluations. In case of insufficient number of respondents the Fischer's exact test was applied. The statistical tests were evaluated at the significance level of 5 %. Results: In our study, it was found out that 22 % of respondents knew exactly what outpatient delivery means, 66 % were not entirely sure and 12 % did not know. 70 % of the medical staff is not in favour of outpatient delivery while 39 % has never had any experience with it. The survey also reveals that 64 % of the participants think that women who undergo outpatient delivery have been informed about possible complications that may arise. 34 % of the participants stated that the internet is the most updated source for that information. For 45 % of the survey contributers, the most common reason for undergoing outpatient delivery is the stress in the hospital environment. Conclusion: The majority of surveyed healthcare professionals do not support outpatient delivery. The reasons, for and against outpatient delivery, that were given by the participants from a pediatric perspective reflected mostly the issues and complications of the newborn, while the gynecological views were addressed mostly to the issues of a woman after giving a birth. All professional groups prefer outpatient delivery rather than giving a birth at home.
