Abstract:

V rámci této bakalářské práce bych chtěla poukázat na světově rozšířenou problematiku cévních mozkových příhod a jejich následků. Prostřednictvím této bakalářské práce bych chtěla seznámit širší veřejnost s existencí tohoto onemocnění. Dále je mou snahou poukázat na rizikové faktory podporující vznik, průběh této diagnózy a možnost prevence před samotným vznikem či recidivám. Název práce: Nácvik každodenních činností u klientů po cévní mozkové příhodě Cíl: Cílem této práce je názorně ukázat na možnosti nácviku jednotlivých činností ADL, které se využívají u klientů po cévní mozkové příhodě při hospitalizaci a tím napomoci k rozvoji daného klienta v testovaných oblastech. Metodika: První oblastí teoretické části je seznámení veřejnosti s problematikou cévní mozkové příhody. Je zde popisován vznik, výskyt v populaci, prevence, typologie, možné následky a komplikace. Tento popis by měl sloužit i laikovi k pomoci v diagnostice příhod a tím dopomoci k rychlejšímu poskytnutí první pomoci a převezení jedince do specializovaných zařízení. V druhé oblasti teoretické části se soustředím na rehabilitaci a možnosti využití ergoterapie při následcích cévní mozkové příhody. V této části se zaobírám otázkou možného navrácení jedince do každodenního života pomocí nácviků běžných denních činností (dále ADL). Praktická část se skládá z přímé kazuistiky klientky, kde popisuji spolupráci, průběh terapie a využívání jednotlivých postupů a testování u tohoto onemocnění. Kazuistika se skládá nejen z průběhu terapie nácviků denních aktivit, ale také se soustředí na otázku odběru anamnézy, vstupního a výstupního vyšetření klientky, určení krátkodobého a dlouhodobého cíle, celkového průběhu ergoterapie a závěrečného hodnocení. Výsledky: Po navázání ergoterapeutického vztahu s klientkou se u vstupního vyšetření získalo značné povědomí o jednotlivých problematických oblastech. Tyto oblasti zahrnují nácvik jemné motoriky, koordinace končetin v prostoru, terapie zaměřené na zacílení, posílení svalové síly, zvětšování rozsahu pohybů a potřebného nácviku soběstačnosti v jednotlivých kategoriích. Těmito kategoriemi jsou funkce sebesycení, hygieny a oblékání. Cílem terapií bylo zlepšení poškozených funkcí a získání maximální míry soběstačnosti. Závěr: V rámci ergoterapeutické intervence při komplexním vstupním vyšetření dále ergoterapeut sestaví krátkodobý a dlouhodobý plán, podporující porušené funkce dané oblasti. Výsledkem terapie v případné kazuistice bylo úplné obnovení funkce dorzální palmární flexe, omezení poklesu ramenního kloubu, a správného stereotypu úchopů. Pozitivním výsledkem terapie bylo zvýšení svalové síly téměř na vyrovnaný stav se zdravou horní končetinou (dále HK). Došlo také k zcela úplnému navrácení jemné motoriky. Díky nácvikům ADL došlo také ke zvýšení maximální soběstačnosti v oblasti sebesycení, hygieny a oblékání. Klientka a její rodinní příslušníci byli po propuštění klientky z hospitalizace edukováni o možnost autorehabilitace v domácím prostředí. Dále jim bylo nabídnuto poradenských služeb při objasnění potřebných informací. Klientka nadále docházela na terapie v rámci ambulantní rehabilitace.

Abstract:

I would like to point out the widespread problem of cerebral vascular events and their consequences in this bachelor thesis. I would like to introduce the existence of this desease to the wider public through this bachelor thesis. In addition, it is my aim to point out the risk factors that support the origin, the course of this diagnosis and the possibility of prevention before the origin or recurrence. Name of the thesis: Practise of everyday activities with patients after cerebral stroke. Aim: The aim of this thesis is to demonstrate the possibilities of practicing individual ADL activities, which are used by clients after stroke during hospitalization and thus help to develop a client in tested areas. Objective: The aim of this work is to acquaint the public with this desease and point out possibilities of occupational therapy intervention during hospitalization. Another aim is to show possibilities of practicing individual activities within the therapy of hospitalized clients after stroke. The goal is also to establish a confidental relationship between the therapist and client and to achieve a maximum reduction in the dependency rate. Methodology: The first area of the theoretical part is to introduction the problém of stroke to the public. There is description of the origin, occurrence in the population, prevention, typology, possible consequences and complications. This description should also serve the laity to assist in the diagnosis of events and thus help to provide first aid and transfer individuals to specializes facilities. The second area of the theoretical part is concentrated on rehabilitation and possibilities of using occupation therapy in the consequences of stroke. In this section I deal with the question of the possible return of an individual to everyday life through activities of daily living (ADL). The practical part consists of a direct case report of a patient, where cooporation, course of therapy, using of individual procedures and testing in this desease are described. The case report is not consisted only of the course of daily activities training, but is also focused on the issue of anamnesis, entry and exit examinations of the patient, determination of the short and long term goal, overall of occupation therapy and final evaluation. Results: After establishing the occupational therapist's relationship with the client, the entrance Examination gained considerable awareness of the individual problem areas. These areas include fine motor training, limb coordination in space, targeting therapies, muscle strength enhancement, increased range of movement and the necessary self-sufficiency training in each category. These categories are functions of self-saturation, hygiene and dressing. The aim of the therapies was to improve impaired functions and to obtain maximum self-sufficiency. Conclusion: Occupational therapist's are going to draw up a short-term and long-term plan supporting the impaired funciton, within the occupational therapy intervention at initial examination. The result of therapy in a case report was a complete restoration of dorsal palmar flexion, a reduction in shoulder joint decline, and a proper grip stereotype. The positive outcome of the therapy was an increase in muscle strength almost to balanced state with a healthy upper limb. There was also a complete recovery of fine motor skills. Self-sufficiency in self-saturation, hygiene and dressing have increased thanks to ADL training. The client and her family members were educated about the possibility of self-rehabilitation at home after release from hospitalization. Furthermore, counseling servisces to clarify the necessary information were offered. The client continued to receive therapies within outpatient rehabilitation.
