NANDA-Int. nursing diagnoses in Home Care

Abstract:

Cíl: Cílem bakalářské práce je využití NANDA diagnostiky v agenturách domácí péče, stanovení nejčastěji se vyskytujících ošetřovatelských diagnóz u klientů v domácí péči a využitelnost hodnotících škál při posouzení klientova stavu a individuálních problémů či potřeb. Dílčím cílem je sestavení sady ošetřovatelských diagnóz pro domácí péči. Bakalářská práce je rozdělena na teoretickou a praktickou část. Teoretická část objasňuje pojmy související se samotnou taxonomií, tak i práci sestry v domácí péči. V praktické části jsou obsaženy výsledky výzkumného šetření. Metodika: Zkoumaný soubor tvořilo 200 klientů (starších 18 let, s lékařsky indikovanou domácí péčí, souhlasící se zapojením ve výzkumném projektu) Agentury domácí péče v Ostravě. Pro sběr ošetřovatelských a demografických dat byl použit speciálně sestavený elektronický protokol, získaná data byla statisticky vyhodnocena a vzájemně korelována prostřednictvím Spearmanova korelačního koeficientu. V situaci, kdy kategorie ke statistickému posouzení zahrnovaly nízkou četnost (pod 5), byl k prokázání statistické významnosti použit Fisherův exaktní test. Výsledky: Celkem bylo diagnostikováno 2470 ošetřovatelských diagnóz (průměr 12,4). Po vyhodnocení získaných dat bylo potvrzeno 41 ošetřovatelských diagnóz s více než 10% četností, které by mohly být zařazeny do základního souboru ošetřovatelských diagnóz pro praktické využití v domácí péči. Mezi diagnosticky nejužívanější metody bylo zařazeno 9 hodnotících škál (test Barthelové, BMI, škála Nortonové, FAQ, nutriční skóre, test kognitivních funkcí, hodnocení psychického stavu, hodnocení stavu deprese a bolesti). Závěr: Kvalitní ošetřovatelská diagnostika, terminologická srozumitelnost a praxe založená na důkazech jsou základní prvky pojetí moderního ošetřovatelství. Abstract:

Objective: The aim of this work is the use of NANDA diagnosis in home care agencies, determining the most frequently occurring diagnoses for nursing home care clients and usability rating scales for assessing the client's condition and individual problems or needs. Partial to build a set of nursing home care diagnoses. The thesis is divided into theoretical and practical. The theoretical part explains the concepts related to the taxonomy itself, as well as the work of nurses in home care. The practical part provides the results of the research. Methods: The sample consisted of 200 clients (over 18 years of home care is medically indicated, consenting to involvement in a research project), home care agencies in Ostrava. For the collection of nursing and demographic data were used specially designed electronic protocol, data were statistically evaluated and correlated with each other through the Spearman correlation coefficient. In a situation where the statistical assessment of the categories included low frequency (below 5) was to establish statistical significance used Fisher's exact test. Results: A total of 2470 diagnosed nursing diagnoses (average 12.4). After evaluating the collected data was confirmed by 41 nursing diagnoses with more than 10% frequency, which could be included in the basic set of nursing diagnoses for practical use in home care. Among the most widely used method of diagnosis were included in the nine rating scales (Barthel test, BMI range of Norton, FAQs, nutritional score, cognitive function, mental status evaluation, assessment of the state of depression and pain). Conclusion: Quality Nursing Diagnosis, terminology and understanding of evidence-based practice are essential elements of the conception of modern nursing.
