Factors of effectiveness of peripartal optimization of anaesthesiological management

Abstract:

Ve své disertační práci se zaměřuji na možnosti optimalizace postupu porodnické anestezie a analgezie s ohledem na bezpečnost rodičky a novorozence. Zejména jsem se soustředila na skupinu rodiček, u kterých je standardní anesteziologický a porodnický postup často modifikován z důvodu obav z dalšího průběhu jejich základního onemocnění – roztroušené sklerózy. Prokázala jsem, že rodičky s roztroušenou sklerózou jsou ohroženy v období 3 měsíců po porodu zvýšenou mírou relapsů, jež však nesouvisí s typem porodu, způsobem vedení anestezie pro císařský řez anebo podáním epidurální porodní analgezie. Metody regionální blokády tedy představují pro rodičky s roztroušenou sklerózou bezpečnou alternativu porodnické anestezie/analgezie. Zkoumala jsem rovněž vliv druhu anestezie u císařského řezu na parametry poporodní adaptace novorozence, zejména pak Apgar skóre. V populaci zdravých donošených novorozenců porozených plánovaným císařským řezem jsem prokázala, že vzhledem k hodnotě Apgar skóre jsou metody celkové a regionální anestezie rovnocenné. Ve třetí části práce jsem se věnovala alternativě epidurální porodnické analgezie, konkrétně intravenózní, pacientkou řízené analgezii za použití ultrakrátce působícího opioidu remifentanilu. Prezentované výsledky naší studie a metaanalýzy potvrdily nejen srovnatelnou bezpečnost a účinnost této metody se zlatým standardem porodnické analgezie – epidurální analgezií, ale i srovnatelnou spokojenost rodiček.

Abstract:

In my dissertation I focus on the possibilities of optimizing the management of obstetric anaesthesia and analgesia with respect to the safety of the mother and the newborn. In particular, I focused on the group of women in whom the standard anesthesia and obstetric procedures are often modified because of concerns about the further course of their underlying disease - multiple sclerosis. I have shown that women with multiple sclerosis are at increased risk of relapses during the 3 months after delivery. However, this is not related to the type of delivery, the way of caesarean section anesthesia, or the administration of epidural birth analgesia. Thus, regional blockade methods are a safe alternative to obstetric anesthesia/analgesia for women with multiple sclerosis. I also investigated the effect of anesthesia in caesarean section on the parameters of postpartum adaptation of the newborn, especially the Apgar score. In the population of healthy in-term newborns born by planned caesarean section, I have shown that the methods of general and regional anesthesia are equivalent due to the Apgar score. In the third part of the thesis I focused on the alternative of epidural obstetric analgesia, namely intravenous, patient-controlled analgesia using the ultra-short-acting opioid remifentanil. The presented results of our study and meta-analysis confirmed not only comparable safety and efficacy of this method with the gold standard of obstetric analgesia - epidural analgesia, but also comparable satisfaction of women.
