The Problems of Patient with Terminal Stage of Amyotrophic Lateral Sclerosis

Abstract:

Cílem bakalářské práce bylo popsat a provést rozbor případu pacientky v terminálním stadiu amyotrofické laterální sklerózy se zmapováním jejich potřeb v průběhu onemocnění se zdůrazněním proměnlivosti zdravotního i psychického stavu pacientky s vyvozením konkrétních opatření pro využití v praxi. Teoretická část je zaměřena na charakteristiku onemocnění, její formy, etiopatogenezi, klinické projevy, diagnostiku a léčbu onemocnění ALS. Praktická část je zaměřena na potřeby pacientky samotné. Vybranou metodou sběru dat byla zvolena kvalitativní výzkumná metoda kazuistiky u záměrně vybrané pacientky s progresivním onemocněním ALS. V rámci kazuistiky je popisován průběh onemocnění a kazuistika je rozdělena do více částí ? anamnézu a katamnézu. Katamnéza popisuje období zjištění diagnózy, období hospitalizace, období zhoršení stavu při hospitalizaci a období umírání pacientky. Kazuistika je zaměřena na bio-psycho-sociální a spirituální problematiku terminálního onemocnění. V závěru bakalářské práce se objevují doporučení pro praxi se zaměřením na potřeby terminálně nemocných, které mohou být přínosné pro péči o pacienty s amyotrofickou laterální sklerózou v terminálním stadiu v podmínkách českého ošetřovatelství.

Abstract:

The aim of this thesis was to describe and analyze the case of a patient with end-stage amyotrophic lateral sclerosis and mapping their needs in the course of the disease with emphasis on the variability of health and psychological state of the patient with the implication of specific measures to use in practice. The theoretical part is focused on the characteristics of the disease, its forms, etiopathogenesis, clinical manifestations, diagnosis and treatment of disease ALS. The practical part is focused on the needs of patients themselves. The chosen method of data collection was a qualitative research method in a case study deliberately selected patients with progressive disease ALS. The case study is described of the disease and a case report is divided into several parts ? anamnesis and katamnesis. Katamnesis describes the diagnosis, hospitalization period, a period of deterioration during hospitalization and the dying patient. Case report focuses on the bio-psycho-social and spiritual problems of terminal disease. The concluding part appear to recommendation for practice focusing on the needs of terminaly ill patients, which may be beneficial for the care of patients with amyotrophic lateral sklerosis in the terminal stage in terms of Czech nursing.
