Molecular biology of head and neck tumors

Abstract:

Incidence nádorů hlavy a krku v České republice ale i ve světě stoupá. Zhruba 80 % těchto nádorů je způsobených vlivy zevního prostředí, zejména kouřením a nadměrnou konzumací alkoholu. Ostatních 20 % je spojeno s přítomností onkogenních lidských papilomavirů (HR HPV), které májí souvislost s určitým druhem sexuálního chování. Cílem této práce je popsat problematiku nádorů hlavy a krku a její spojitost s HPV infekcí, zjistit prevalenci a typovou specifitu HPV a zda se navzájem klinicky, anebo demograficky liší pacienti dle HPV pozitivity či negativity. Nakonec stanovit metylační stav vybraných genů. Celková prevalence HPV DNA byla 47,2 % a téměř většina těchto nádorů se vyskytovala v orofaryngeální oblasti. HPV negativní nádory se vyskytovaly v dutině ústní a tito pacienti byli častěji kuřáci nebo bývalí kuřáci. Metylační status byl stanoven u genu CDKN2A/p16 a ACTB, který sloužil jako kontrolní vzorek. U genu CDKN2A/p16 byla detekována hypermetylace u HPV negativních nádorů. U HPV pozitivních nádorů nebyla ve většině případů metylace p16 genu vůbec detekována. Výsledky této studie potvrzují různorodost mezi HPV pozitivními a HPV negativními nádory v oblasti hlavy a krku.

Abstract:

The incidence of head and neck tumors is rising in the Czech Republic but also in the world. About 80 % of these tumors are caused by environmental influences, especially smoking and excessive alcohol consumption. The other 20 % are associated with the presence of oncogenic human papillomaviruses (HR HPV), which are associated with some type of sexual behavior. The aim of this work is to describe the problem of head and neck tumors and its connection with HPV infection, to determine the prevalence and type specificity of HPV and whether patients differ clinically or demographically in patients according to HPV positivity or negativity. Finally, determine the methylation status of selected genes. The overall prevalence of HPV DNA was 47,2 % and almost most of these tumors occurred in the oropharyngeal region. HPV-negative tumors occurred in the oral cavity, and these patients were more likely to be smokers or ex-smokers. Methylation status was determined for the CDKN2A/p16 and ACTB genes, which served as a control. For the CDKN2A/p16 gene, hypermethylation was detected in HPV-negatives tumors. In most cases, methylation of the p16 gene was not detected at all in HPV-positive tumors. The results of this study confirm the diversity between HPV-positive and HPV-negative head and neck tumors.
