Abstract:

Úvod: Vysokofrekvenční audiometrie (VFA) je velmi užitečný test, který dokáže detekovat ztrátu sluchu dříve, než se projeví na středních a nízkých frekvencích, které významně ovlivňují kvalitu sluchu. Pro osoby s otologicky fyziologickým nálezem jsou stanoveny doporučené prahy sluchu s ohledem na věk a pohlaví v normě ČSN EN ISO 7029:2017 pro frekvence od 0,125 kHz do 12,5 kHz. V této normě jsou pouze odhadované hodnoty prahů sluchu na vysokých frekvencích (VF) od 9 kHz do 12,5 kHz a pro frekvenci 14 kHz a 16 kHz nejsou hodnoty prahů sluchu stanoveny vůbec. Cíl: Cílem práce je zjistit práh sluchu pomocí konvenční audiometrie (KA) a VFA u otologicky zdravé populace v produktivním věku neexponované hlukem na pracovišti a nalezení možných rizikových faktorů pro vznik poruchy sluchu. Metodika: Studie byla provedena formou observační deskriptivní průřezové studie (cross-sectional study), ve které byly získávány informace dotazníky a měřením sluchového prahu respondentů. Do studie byli zařazeni jedinci z běžné populace, kteří neměli nikdy problémy se sluchem a nebyli exponováni riziku hluku na pracovišti. Probandi byli rozděleni dle věku do 5 kategorií: od 18 do 24 let a dále po 10ti letech. Každá z těchto skupin byla dále rozdělena dle pohlaví. Všichni jedinci podstoupili tympanometrii, KA ve frekvenčním rozsahu od 0,125 kHz do 8 kHz a VFA ve frekvenčním rozsahu od 9 kHz do 16 kHz. Pro statistické zpracování byla stanovena 5% hladina významnosti. Výsledky: Byly stanoveny prahy sluchu u otologicky zdravé populace ve vysokofrekvenčním pásmu od 9 kHz do 16 kHz. VFA je vysoce citlivou metodou pro včasnou detekci ztráty sluchu, jelikož se VF prahy sluchu začínají zvyšovat již od věku 35+. U mužů se zvyšovaly prahy sluchu s věkem rychleji než u žen. Schopnost probandů reagovat na VFA postupně klesala s věkem a rostoucí frekvencí až na 34 %. Hlasitost poslechu osobních poslechových zařízení (OPZ) byla nejvýznamnějším rizikovým faktorem pro vznik nedoslýchavosti, posluchači OPZ měli 3,44krát vyšší šanci vzniku nedoslýchavosti. Závěr: Výsledky studie doplnily znalosti o VF prazích sluchu u mužů a žen vzhledem k věku. Výsledky by mohly být zdrojem normativních dat pro otologicky zdravou středoevropskou populaci. Používání OPZ je významným rizikovým faktorem pro vznik nedoslýchavosti. VFA lze považovat za vysoce citlivou metodu pro hodnocení ztráty sluchu v závislosti na věku a pohlaví. Včasná diagnostika a léčba nedoslýchavosti by mohla pomoci mnoha nedoslýchavým co nejdéle udržet dobrou kvalitu života.

Abstract:

Introduction: High Frequency Audiometry (HFA) is a very useful test that can detect hearing loss before it occurs at medium and low frequencies that have a significant impact on a quality of hearing. For persons with otologically physiological hearing, the recommended hearing thresholds considering age and gender are set in the standard ČSN EN ISO 7029: 2017 for the frequency from 0.125 kHz up to 12.5 kHz. The standart contains only the estimated values of hearing thresholds at high frequencies (HF) from 9 kHz up to 12.5 kHz and hearing threshold for the frequency of 14 kHz and 16 kHz are not established at all. The aim: The aim of this thesis is to determine the hearing threshold using conventional and high-frequency audiometry in an otologically healthy population of productive age that is not exposed to noise in the workplace and to find possible risk factors for hearing loss. Methodology: The study was accomplished in the form of an observational descriptive cross-sectional study, in which information was obtained by questionnaires and by measuring the hearing thresholds of the respondents. The study included individuals from the general population who had never had a hearing problem and were not exposed to the risk of noise in the workplace. Probands were divided into 5 categories according to their age: from 18 to 24 years and further by 10 years of age. Each of these groups was further divided by gender. All subjects underwent tympanometry, conventional audiometry in the frequency range from 0.125 kHz to 8 kHz, and high-frequency audiometry in the frequency range from 9 kHz to 16 kHz. A 5% level of significance was determined for statistical processing. Results: Hearing thresholds in an otologically healthy population on a frequency range from 9 kHz to 16 kHz were established. HFA is a highly sensitive method for early detection of hearing loss, as HF hearing thresholds begin to increase from the age of 35+. For men, hearing thresholds increased faster with age than for women. The ability of probands to respond to HFA gradually decreased with age and with the increasing frequency down to 34%. The volume of listening to personal listening devices (PLD) was the most significant risk factor for hearing loss, PLD listeners had a 3.44 times higher chance of hearing loss. Conclusion: The results of the study could contribute the knowledge about HF hearing thresholds of men and women with respect to age. The results could be a source of normative data for an otologically healthy Central European population. The use of PLDs is a significant risk factor for hearing loss. HFA can be considered a highly sensitive method for assessing hearing loss considering age and gender. Early diagnosis and treatment of hearing loss could help many hearing-impaired people maintain a good quality of life for as long as possible.
