Abstract:

Genetické abnormality u pacientů s psychomotorickou retardací detekované pomocí molekulárně cytogenetických metod Mentální retardace je onemocnění vyskytující se u 2 - 3 % populace. Přestože se díky novým laboratorním metodám daří odhalit příčinu tohoto postižení u více jedinců než dříve, zůstává stále až 50 % případů neobjasněno. Tato diplomová práce byla zaměřena na soubor 100 pacientů s MR, diagnostikovaných na OLG FN Brno v letech 2007 a 2008. U všech pacientů byl stanoven karyotyp pomocí G-pruhování, molekulárně cytogenetické analýzy byly provedeny pomocí technik HR-CGH, MLPA, FISH a aCGH. Z celkového počtu 100 pacientů bylo nalezeno 16 (16 %) aberací, konkrétně 11 chromozomových změn pomocí G-pruhování a 5 aberací odhalených až pomocí molekulárně cytogenetických metod. U pacientů s nálezem v karyotypu byla potvrzena a upřesněna aberace v osmi případech: pacient č. 9: 46,XX,dup(12)(p11.2p12); pacient č. 12: 47,XY,dup(15)(q11.1q14); pacient č. 14: 46,XX,dup(6)(q16q21); pacient č. 29: 46,XX,dup(3)(p21p22); pacient č. 35: 47,XY, i(18)(p10pter); pacient č. 56: 46,XY,del(7)(q35qter),dup(4)(q32qter); pacient č. 58: 46,XY, dup(20)(p11.2); pacient č. 99: mozaika 47,XX, +dup(9)(q12q22)[7]/46,XX[3] U pacientů s normálním karyotypem z celkového počtu 89 (89 %) byla nalezena aberace u 5 (5,5 %) z nich. Pomocí technik HR-CGH, MLPA, FISH a aCGH byla upřesněna poloha a velikost aberace. U pacienta č. 32 se jednalo o deleci (21)(q22.2qter) o velikosti 6 Mb a duplikaci (11)(q24.2qter) o velikosti 8 Mb; u pacienta č. 36 byla nalezena delece (1)(p36.33pter) o velikosti 2,2 Mb; u pacienta č. 47 se jednalo o deleci (1)(p36.23pter) o velikosti 7,3 Mb; u pacienta č. 54 byla prokázána delece (6)(q14.2q15) o velikosti 7 Mb; u pacienta č. 61 se jednalo o duplikaci (1p). Předložená práce prokázala, že analýzy genomu pomocí moderních molekulárně cytogenetických metod významným způsobem posunují citlivost určování genetických změn, což má velký význam i při odhalování genetických příčin MR.

Genetic abnormalities in patients with mental retardation studied using molecular cytogenetic methods Mental retardation is a disease occurring in 2 – 3 % of population. Despite of utilization of new laboratory methods, which allow more effective investigation and thus higher rate of successfully closed cases, there still remain about 50 % of cases which cannot be clarified. The main aim of this diploma thesis was investigation of chromosomal changes in a cohort of 100 patients with mental retardation diagnosed in OLG FN Brno during 2007 – 2008. The analysis of karyotypes using G-banding was performed in all patients. For more precise characterisation of chromosomal aberrations HR-CGH, MLPA, FISH and array-CGH technique were used. In group of 100 patients were determined 16 cases (16 %) with some chromosomal aberration, from which 11 was detected by G-banding and 5 by using other molecular cytogenetic methods. In patients with positive finding in karyotype were by G-banding verified and specified certain chromosomal aberrations in 8 cases: patient no. 9: 46,XX,dup(12)(p11.2p12); patient no. 12: 47,XY,dup(15)(q11.1q14); patient no. 14: 46,XX,dup(6)(q16q21); patient no. 29: 46,XX,dup(3)(p21p22); patient no. 35: 47,XY, i(18)(p10pter); patient no. 56: 46,XY,del(7)(q35qter),dup(4)(q32qter); patient no. 58: 46,XY,dup(20)(p11.2) and patient no. 99: mosaic 47,XX, +dup(9)(q12q22)[7]/46,XX[3]. In 89 patients with normal karyotype some chromosomal aberration was found in 5 cases (5,5 %) using molecular cytogenetic techniques. Location and size of aberration were specified using HR-CGH, MLPA, FISH and array-CGH techniques. In patient no. 32 deletion 6 Mb was detected at (21)(q22.2qter) and duplication 8 Mb was detected at (11)(q24.2qter); in patient no. 36 deletion 2,2 Mb was found at (1)(p36.33pter); in patient no. 47 deletion 7,3 Mb was detected at (1)(p36.23pter); in patient no. 54 deletion 7 Mb was proved at (6)(q14.2q15) and in patient no. 61 duplication was found (1p). This diploma thesis showed that genomic analyses using advanced molecular cytogenetic methods fundamentally improve the sensitivity of detection of chromosomal changes. Therefore methods of molecular cytogenetics are significant tools in determination of genetic causes in patients with mental retardation.
