Abstract:

Souhrn: Diplomová práce se zabývá hodnocením autonomní regulace kardiovaskulárního systému u osob s roztroušenou sklerózou mozkomíšní měřenou sekvenční metodou baroreflexní senzitivity při spontánním a řízeném dýchání podle metronomu (f = 0,33 Hz). Byl vytvořen homogenní soubor o počtu 25 pacientek (věk 44,1 ± 8,1 let, délka onemocnění 6,2 ± 5,7 let, míra klinického postižení 3,0 ± 1,6). Dle prvního kritéria (délka onemocnění) byly pacientky nejprve rozděleny na 3 soubory. Soubor A tvořilo 7 žen (47,7 ± 7,5 let, délka onemocnění 0,7 ± 0,3 let, míra klinického postižení 2,6 ± 0,7), soubor B 12 žen (41,4 ± 9,1 let, délka onemocnění 5,5 ± 2,5 let, míra klinického postižení 3,1 ± 2,0) a soubor C 6 žen (45,2 ± 5,5 let, délka onemocnění 14,2 ± 4,7 let, míra klinického postižení 3 ± 1,9). Dle druhého kritéria dělení (míra klinického postižení) byly vytvořeny dva soubory. Soubor A tvořilo 15 žen (43,2 ± 7,4 let, délka onemocnění 5,2 ± 5,2 let, míra klinického postižení 1,9 ± 0,7) a soubor B tvořilo 10 žen (45,4 ± 9,3 let, délka onemocnění 7,8 ± 6,4 let, míra klinického postižení 4,8 ± 0,8). Získané hodnoty baroreflexní senzitivity (BRS) při spontánním a řízeném dýchání dle metronomu (f = 0,33 Hz) byly u vybraného vzorku (n = 25) pacientek srovnány s normohodnotou (9,3 ms/mm Hg) odpovídající zdravé populaci. Následně byly získané hodnoty BRS při řízeném dýchání srovnány napříč soubory v rámci daného kritéria. Nebyly zjištěny statisticky významné rozdíly v naměřených hodnotách BRS.

Summary: This thesis deals with the Evaluation of Autonomic Regulation of the Cardiovascular System in patiens with multiple sclerosis measured by the sequential baroreflex sensitivity method during spontaneous and metronome controlled breathing (f = 0,33 Hz). A homogeneous set of 25 patients (age 44,1 ± 8,1 years of age, disease duration of 6,2 ± 5,7 years, degree of clinical impairment of 3,0 ± 1,6) has been created. According to the first criterion (disease duration) we divided the patiens into 3 files. Group A consisted of 7 women (47,7 ± 7,5 years of age, disease duration of 0,7 ± 0,3 years, degree of clinical impairment of 2,6 ± 0,7), group B had 12 women (41,4 ± 9,1 years of age, disease duration of 5,5 ± 2,5 years, degree of clinical impairment of 3,1 ± 2,0) and the group C contained 6 women (45,2 ± 5,5 years of age, disease duration of 14,2 ± 4,7 years, degree of clinical impairment of 3,0 ± 1,9). The second criterion (clinical degree of disability) divided the patients into two files. Group A consisted of 15 women (43,2 ± 7,4 years of age, disease duration of 5,2 ± 5,2 years, degree of clinical impairment of 1,9 ± 0,7) and group B contained 10 women (45,4 ± 9,3 years of age, disease duration of 7,8 ± 6,4 years, degree of clinical impairment of 4,8 ± 0,8). We compared the values measured by the sequential baroreflex sensitivity method during spontaneous and metronome controlled breathing by the chosen set of 25 patients with a standardized value (9,3 ms/mm Hg) corresponding to the healthy population. Subsequently, we compared the measured values across files within a given criteria. We did not find any statistically significant differences in measured BRS values.
