Nursing Process in a Patient after Lung Transplantation

Abstract:

Tématem bakalářské práce je Ošetřovatelský proces u pacienta po transplantaci plic. Hlavním cílem bakalářské práce je popis a analýza případu pacienta po transplantaci plic kvůli progresivnímu plicnímu onemocnění hospitalizovaného na jednotce intenzivní péče. Teoretická část se zabývá charakteristikou transplantace plic, kde je věnována pozornost na druhy transplantace plic, indikace a kontraindikace k zařazení na čekací listinu. Mimo výše uvedené je také věnována pozornost na odběr a dárce plic, předoperační a perioperační péči, popis operačního výkonu, princip extrakorporální membránové oxygenace a pooperační komplikace. Dále je kapitola zabývající se pooperační ošetřovatelskou péčí o pacienta po transplantaci plic na anesteziologicko-resuscitačním oddělení, jednotce intenzivní péče, standardním oddělení a následnou ambulantní péčí. Třetí kapitola z teoretické části se zaměřuje na život transplantovaného pacienta po propuštění do domácího prostředí. Praktická část bakalářské práce se zabývá ošetřovatelským procesem u pacienta po transplantaci plic. V této části práce byl nejprve posouzen celkový stav pacienta, následně byly ošetřovatelské problémy převedeny na ošetřovatelské diagnózy dle NANDA I Taxonomie II. Dle stanovených cílů byly naplánovány intervence, které se následně zrealizovaly. Ošetřovatelský proces byl posléze vyhodnocen. V závěru práce je zmíněné doporučení pro praxi určené pro všeobecné sestry, pacienty po transplantaci plic a jejich rodiny.

Abstract:

The topic of this bachelor's thesis is the Nursing Process in a Patient after a Lung Transplantation. The main aim of this bachelor's thesis is to describe and analyse the case of a lung transplant patient who is hospitalised in an intensive care unit due to a progressive lung disease. The theoretical part deals with the characteristics of lung transplantation, where attention is paid to the description of lung transplantation types, indications and contraindications for inclusion on the waiting list. In addition to the above, attention is also paid to lung donation and donors, preoperative and perioperative care, surgery description, principle of extracorporeal membrane oxygenation and postoperative complications. There is also a chapter dealing with postoperative nursing care of a patient after lung transplantation in anaesthesiology and resuscitation ward, intensive care unit, standard ward and subsequent outpatient care. The third chapter of the theoretical part focuses on the life of the transplant patient after being discharged to their home environment. The practical part of the thesis deals with the nursing process in the patient after a lung transplantation. In this part, the patient’s overall condition was first assessed, and then the nursing problems were transferred to nursing diagnoses according to NANDA I Taxonomy II. Interventions were planned according to the set objectives and subsequently implemented. The nursing process was then evaluated. The thesis is concluded with recommendation for practice intended for general nurses, lung transplant patients and their families.
