Effect of obesity on spirometric, anthropometric and strength parameters, dyspnoea rate and exercise tolerance in patients with bronchial asthma

Abstract:

V rámci této diplomové práce jsou zkoumány souvislosti mezi onemocněním astma bronchiale a obezitou u dospělých jedinců. Teoretická část je rozdělena do tří kapitol, kdy první shrnuje poznatky o onemocnění astma bronchiale od definice, přes etiopatogenezi, klasifikaci až po léčbu a prevenci v podobě stravy. Druhá kapitola je věnována obezitě, její definici, etiopatogenezi, rizikovým faktorům a komplexní léčbě včetně pohybové aktivity. Třetí kapitola přibližuje souvislosti mezi těmito onemocněními, popisuje dva fenotypy obézních astmatiků, vliv genetiky, ovlivnění funkce plic, a také léčbu a úlohu stravy. Cílem této práce bylo zhodnotit vliv obezity na spirometrické, antropometrické a silové parametry, míru dušnosti a tolerance fyzické zátěže u nemocných s astma bronchiale. Sledovaný soubor tvořilo 16 probandů (8 žen, 8 mužů, věk 52,3 ? 15,2 let, výška 172,2 ? 10,1 cm, hmotnost 90,4 ? 18,9 kg, BMI 30,5 ? 5,9 kg/m2) nemocných s astma bronchiale. U každého z probandů bylo provedeno měření k získání vybraných parametrů. Pro zhodnocení tělesné kompozice byla použita profesionální váha značky TANITA DC-240. Bylo provedeno antropometrické měření hodnotící rozvíjení hrudníku a obvod krku. Byla zjištěna míra dušnosti dle mMRC a tolerance fyzické zátěže, která byla měřena pomocí Sit to Stand testu. Dále byla získána dynamometrem síla stisku na horních končetinách a naměřeny maximální ústní tlaky MIP a MEP. Vztahy mezi proměnnými byly zkoumány pomocí regresní analýzy. Párovou regresí byl zjištěn statisticky významný vliv u některých parametrů obezity a rozvíjením hrudníku, sílou dechových svalů a sílou stisku na horních končetinách. Naopak nebyl zjištěn u sledovaného souboru statisticky významný vliv na toleranci fyzické zátěže a na míru dušnosti.

Abstract:

This diploma thesis investigates the association between bronchial asthma and obesity in adults. The theoretical part is divided into three chapters, where the first one summarizes the knowledge about bronchial asthma from definition, etiopathogenesis, classification to treatment and dietary prevention. The second chapter is devoted to obesity, its definition, etiopathogenesis, risk factors and complex treatment including physical activity. The third chapter reviews the relationship between these diseases, describing the two phenotypes of obese asthmatics, the influence of genetics, the impact on lung function, and the treatment and role of diet. The aim of this thesis was to evaluate the effect of obesity on spirometric, anthropometric and strength parameters, dyspnoea rates and exercise tolerance in patients with bronchial asthma. The study population consisted of 16 probands (8 females, 8 males, age 52.3 ? 15.2 years, height 172.2 ? 10.1 cm, weight 90.4 ? 18.9 kg, BMI 30.5 ? 5.9 kg/m2) of patients with bronchial asthma. Measurements were performed in each proband to obtain selected parameters. A TANITA DC-240 professional scale was used to assess body composition. Anthropometric measurements were taken to assess chest expansion and neck circumference. The level of dyspnea according to mMRC and exercise tolerance was measured using Sit to Stand test. In addition, upper limb grip strength was obtained with a dynamometer and maximum oral pressures MIP and MEP were measured. Relationships between variables were examined using regression analysis. Pairwise regression revealed a statistically significant effect for some obesity parameters and chest expansion, respiratory muscle strength and upper limb grip strength. On the other hand, no statistically significant effect was found for exercise tolerance and dyspnoea rate in the study population.
