The effect of the revision total hip replacement on the symmetry of lower limbs loading during gait

Abstract:

Cílem této práce bylo určit vliv revizní operace totální náhrady kyčelního kloubu na zatížení dolních končetin při chůzi. Sledovali jsme rozdíly mezi operovanou a neoperovanou dolní končetinou a také mezi skupinami podle doby uplynulé od revizní operace. Výzkumný soubor tvořilo 24 pacientů Ortopedické kliniky Fakultní nemocnice v Olomouci (12 mužů, 12 žen: průměrný věk 60,3?7,4 roku, výška 167,8?10,2 cm, hmotnost 78,4?16,9 kg, průměrná doba po operaci 7,2?4,2 roku). Všichni probandi byli minimálně jeden rok po jednostranné revizi totální náhrady kyčelního kloubu, kde byl použit transkutánní Bauerův přístup. Pro měření zatížení dolních končetin při chůzi byla použita plantografická plošina Footscan?. Zjistili jsme významně větší zatížení v oblasti mediální části paty a středonoží na operované dolní končetině (p ? 0,05). Při porovnávání skupin s různou délkou od prodělané operace jsme nalezli významné rozdíly (p ? 0,05) v zatížení v oblasti laterální paty, v oblasti středonoží a 1., 2. a 3. metatarzu. Zatížení bylo u prvních dvou zmíněných oblastí chodidla větší u skupin s kratší dobou od operace, zatížení metatarzů bylo větší u skupin s delší dobou od operace. Z výsledků práce vyplývá důležitost obnovy správného stereotypu chůze a z toho plynoucí rovnoměrné zatížení obou dolních končetin u pacientů po výměně kyčelního kloubu.

Abstract:

The aim of this thesis was to evaluate the effect of reperformed hip replacement surgery on lower limb stress. The difference between the operatedand non-operated lower limb was compared. The groups were compared according to the time passed since the surgery. The examined group was composed of 24 patients of Orthopaedic Clinic in Olomouc Faculty Hospital (12 men, 12 women: average age 60,3?7,4 years; height 167,8?10,2; weight 78,4?16,9; average time since the surgery 7,2?4,2 years). The minimum time since the reperformed surgery in all patients was 1 year. The Bauer transcutaneous approach was appplied in all cases. The plantographic board Footscan? was used to measure the lower limb stress. Significantly bigger stress was found on the medial side of heel and middle part of the foot on the operated lower limb (p<0,05). We found big differences in stress on the lateral side of the heel, the middle part of the foot and 1st, 2nd and 3rd metatarsal bone while comparing the groups with different time since the hip surgery. The stress on the first two areas was bigger in groups with shorter time since the surgery. The stress on 1st, 2nd and 3rd metatarsal bone was bigger in groups with longer time since the hip surgery. The results are showing the importance of rebuilding the correct stereotype of walking and therefore equal loading of lower limbs after hip replacement surgery.
