The Significance of the Sentinel Lymph Node Biopsy Following the Neoadjuvant Therapy in Patients with Breast Cancer

Abstract:

Úvod: Karcinom prsu je nejčastěji diagnostikovaným tumorem u žen, tvoří 25 % všech nádorových onemocnění. Zavedením neoadjuvantní chemoterapie (NAC) se u pacientek s karcinomem prsu objevila nová možnost léčby. Vyvstalo ale mnoho nových otázek, a to zvláště v oblasti operačních metod v axile. Disertační práce shrnuje v teoretické části nejdůležitější informace a aktuální studie týkající se tématu operační léčby karcinomu prsu po neoadjuvantní terapii s bližším zaměřením na oblast axily. Metody: Klinická část disertační práce je založena na datech získaných prospektivní multicentrickou studií, která probíhala od 1.1.2018 do 30.11.2020 ve třech mamárních centrech v ČR. Primárním cílem studie bylo zjištění falešné negativity chirurgické biopsie sentinelové uzliny (SLNB) u pacientek s karcinomem prsu a cN1 před NAC a ycN0 po NAC. Sekundárním cílem studie bylo zjistit, zda je možné při pozitivitě sentinelové uzliny (SLN) odstraňovat pouze I. etáž axilárních uzlin bez ovlivnění onkologické bezpečnosti. Terciárním cílem bylo srovnání četnosti komplikací a lokoregionálních recidiv po různých typech operačních výkonů v axile. Výsledky: Falešná negativita SLNB u skupiny pacientek cN1 před NAC a ycN0 po NAC byla 12,5 %. Při pozitivitě sentinelové uzliny u pacientek po NAC se u 66,7 % vyskytovaly v II. etáži axilárních uzlin lymfatické uzliny s nádorovou infiltrací, u 41,7 % pacientek by při disekci pouze I. etáže axilárních uzlin došlo k understagingu. Falešná negativita peroperačního histologického vyšetření SLN je 38,5 %. Incidence komplikací ve skupině pacientek s axilární disekcí I. etáže, cílenou axilární disekcí (TAD) nebo samplingem byla 18,5 %, ve skupině s axilární disekcí I. a II. etáže 29,0 %, mezi skupinami nebyla prokázána statistická významnost (P = 0,3784). Souvislost lokoregionální recidivy a metody použité při operaci v axile nebylo možné hodnotit pro nízký počet případů. Závěr: Falešná negativita SLNB u skupiny pacientek cN1 před NAC a ycN0 po NAC přesahuje tolerovatelnou hranici 10 %. Disekce I. etáže axilárních uzlin je v případě pozitivní sentinelové uzliny u pacientek po NAC nedostatečně onkologicky radikální metodou. Falešná negativita peroperačního histologického vyšetření SLN je 38,5 %. Incidence komplikací byla vyšší u skupiny s axilární disekcí I. a II. etáže, ale nebyla statisticky významná ve srovnání s ostatními operačními výkony v axile.

Abstract:

Introduction: Breast cancer is the most common tumour in women, accounting for 25% of all cancers. With the introduction of neoadjuvant chemotherapy (NAC), a new treatment option has emerged in patients with breast cancer. However, many new questions have arisen, especially in the field of surgical methods in the axilla. The theoretical part of this thesis summarizes the most important information and current studies on the topic of surgical treatment of breast cancer after the neoadjuvant therapy with a special focus on the axilla. Methods: The clinical part of the presented thesis is based on data obtained by a prospective multicentre study, which took place from 1.1.2018 to 30.11.2020 in three departments specialising in breast cancer surgery in the Czech Republic. The primary objective of the study was to determine the false negativity rate of sentinel lymph node biopsy (SLNB) in patients with breast cancer and cN1 before NAC and ycN0 after NAC. The secondary aim of the study was to determine whether it is possible to remove only the level I axillary nodes if the sentinel node (SLN) is positive without affecting the oncological safety. The tertiary goal was to compare incidence of complications and locoregional recurrences after different surgical methods in the axilla. Results: The false negativity rate of the SLNB in the cN1 group of patients before NAC and ycN0 after NAC was 12.5% in our trial. In patients after NAC with sentinel lymph node positivity, tumour infiltration of level II axillary lymph nodes occurred in 66.7% of cases, 41.7% of patients with level I axillary dissection only would be understaged. The false negative rate of perioperative histological examination of the SLN is 38.5%. The incidence of complications in the group of patients with the level I axillary dissection, targeted axillary dissection (TAD) or sampling was 18.5%, in the group with level I and II axillary dissection 29.0%. No statistical significance was detected between the groups (P = 0.3784). The relationship between the locoregional recurrence and the method used in axillary surgery could not be assessed due to a low number of cases. Conclusion: The false negative rate of SLNB in the group of patients with cN1 before NAC and ycN0 after NAC exceeds the tolerable limit of 10%. In the case of a positive sentinel node, dissection of the level I axillary nodes cannot be considered to be of sufficient oncological radicality in patients after NAC. The false negative rate of perioperative histological examination of the SLN is 38.5%. The incidence of complications was higher in the group with level I and II axillary dissection, but the difference was not statistically significant compared to other surgical methods in the axilla.
