Fyziotherapy after total hip arthroplasty

Abstract:

Cíle: Cílem této bakalářské práce je přispět k řešení problematiky fyzioterapie po totální endoprotéze kyčelního kloubu. Metodika: V této bakalářské práci jsem z hlediska metodologie použila rozhovor pro odebrání anamnézy, pozorování, testování a měření. Terapie probíhala s pacientem po totální endoprotéze kyčelního kloubu. Provedla jsem vstupní kineziologický rozbor, který se skládal z vyšetření aspekcí, palpací, vyšetření zkrácených svalů a pohybových stereotypů, z dynamického vyšetření páteře, vyšetření pánve a kyčelního kloubu, vyšetření stoje a chůze. Na základě kineziologického rozboru jsem stanovila krátkodobý fyzioterapeutický plán. Každá terapie trvala 30-45 minut, 1x - 2x týdně, po dobu pěti týdnů. V průběhu terapií jsem použila metody a koncepty např. techniky měkkých tkání, dechovou gymnastiku, pasivní a aktivní pohyby, postizometrickou relaxaci, Senzomotorickou stimulaci a pomůcky jako jsou Thera - band, overball. Při čtvrté terapii jsem provedla kontrolní vyšetření a po ukončení terapie jsem na základě výstupního kineziologického rozboru navrhla dlouhodobý fyzioterapeutický plán. Výsledky: V průběhu terapií došlo ke snížení otoku v oblasti stehna nad patellou o 1 cm, rozsah pohybu v kyčelním kloubu se zlepšil do flexe o 40°, extenze o 5° a abdukce o 10° od kontrolního vyšetření. Zároveň došlo ke zlepšení abdukce o 5° v porovnání se vstupním vyšetřením, které proběhlo před operací. V kolenním kloubu se zvýšil rozsah do flexe o 40° od kontrolního vyšetření. Svalová síla se zlepšila v kyčelním kloubu do flexe ze stupně 4- na 4, extenze ze stupně 3 na 4- a vnitřní rotace ze stupně 3 na 4- od vstupního vyšetření. Dále se podařilo zlepšit stereotyp chůze o dvou podpažních berlích. Závěr: V rámci krátkodobého plánu jsme si stanovili cíle fyzioterapie, které se mi v průběhu spolupráce s pacientem podařilo zrealizovat.

Abstract:

Aims: The aim of this bachelor thesis is to contribute to solving the problem of physiotherapy after total hip arthroplasty. Methodology: In this bachelor thesis, in terms of methodology, I used interview to take history, observation, testing and measurement. The therapy was conducted with a patient after total hip arthroplasty. I performed an initial kinesiological analysis, which consisted of an examination of aspects, palpation, examination of shortened muscles and movement patterns, dynamic spinal examination, pelvic and hip joint examination, and standing and gait examination. On the basis of the kinesiological analysis, I established a short-term physiotherapy plan. Each therapy session lasted 30-45 minutes, once or twice a week for five weeks. During the therapies I used methods and concepts such as soft tissue techniques, breathing gymnastics, passive and active movements, postisometric relaxation, Sensomotor stimulation and aids such as Thera - band, overball. During the fourth therapy I performed a follow-up examination and after the end of the therapy I designed a long-term physiotherapy plan based on the outcome kinesiological analysis. Results: During the course of therapy, the swelling in the thigh above the patella was reduced by 1 cm, the range of motion in the hip joint improved to 40° of flexion, 5° of extension and 10° of abduction from the follow-up examination. At the same time, abduction improved by 5° compared with the initial preoperative examination. In the knee joint, the range into flexion increased by 40° from the control examination. Muscle strength improved in the hip joint into flexion from grade 4- to 4, extension from grade 3- to 4-, and internal rotation from grade 3- to 4- from the initial examination. Furthermore, the stereotype of walking on two underarm crutches was improved. Conclusion: in the framework of the short-term plan, we set goals for physiotherapy, which I managed to implement during the collaboration with the patient.
