Safety of patients with fractures of the femur during intraoperative care

Abstract:

Cíl: Cílem práce bylo zjistit jak je zajištěná bezpečnost pacienta se zlomeninou stehenní kosti v průběhu operačního výkonu z pohledu anesteziologické a perioperační sestry. Na základě získaných dat navrhnou vhodné opatření vedoucí ke zlepšení bezpečnosti pacienta během pobytu na operačním sále. Metodika: Soubor tvořilo 92 perioperačních a anesteziologických sester Fakultní nemocnice Ostrava a Městské nemocnice Ostrava, p. o. Dotazníkové šetření bylo realizováno v období od prosince 2010 do února 2011. Zjištěná data jsme zpracovali pomocí tabulek a grafů v programu Microsoft Excel. Otázky byly uzavřené s výběrem jedné alternativy odpovědi. Výsledky: Šetřením na výše uvedených pracovištích nebyl zjištěn rozdíl, v četnosti použití extenční polohy u pacientů se zlomeninou stehenní kosti. Dále bylo zjištěno, že v uvedení pacienta do požadované polohy a vedením záznamu o této poloze jsou mezi pracovišti určité rozdíly. Ve FNO se podílí na této činnosti pomocný personál, perioperační sestry a lékaři, v MNO provádí tuto činnost výhradně pomocný personál. V otázkách použití antidekubitálních pomůcek byly výsledky srovnatelné, lišily se odpovědi na jejich dostatek a druh. V otázkách týkajících se výskytu dekubitů během operačního výkonu jsou výsledky poměrně alarmující na obou pracovištích. Taktéž byly zjištěny nedostatky v nutnosti hlášení výskytu dekubitů a jeho formě. Závěr: Na zajištění bezpečnosti pacienta během operačního výkonu by se měl podílet všechen personál operačního sálu. Zvýšením kvality intraoperační péče a důsledným vedením dokumentace během pobytu pacienta na operačním sále, bude snížení výskytu dekubitů a prevence rizik spojených s tímto pobytem. Abstract:

Objective: The aim was to determine how is ensured patient safety with a fractured thigh bone during the surgery from the viewpoint of anesthesia and perioperative nurses. Based on the data to propose appropriate precaution to improve patient safety during their stay in the operating room. Methods: The cohort included 92 perioperative and anesthetic nurses from departments of Central operating rooms in University Hospital Ostrava and City Hospital Ostrava. The questionnaire survey was realized in the period from December 2010 to February 2011. The observed data are processed using tables and graphs in Microsoft Excel. Questions have been closed with the selection of one alternative answers. Results: The investigation of the above mentioned departments was no difference in frequency of use extention position in patients with fractures of the femur. It was found that placing the patient in the desired position and record-keeping for this position are some differences between departments. In University Hospital Ostrava are involved in this work support staff, perioperative nurses and doctors. In City Hospital Ostrava carry out such activities only support staff. In matters concerning the use of antidecubital aids results were similar but differ in their response about kind and enough. In matters relating to the occurrence of pressure ulcers during the surgical procedure the results are quite alarming in both the workplace. We also identified weaknesses in the need for reporting of pressure sores and its form. Conclusion: To ensure patient safety during the surgery should be involved in all operating room personnel. Increasing the quality of intraoperative care about thorough documentation of leadership during the stay of a patient in the operating room to reduce the incidence of pressure ulcers about prevention of risks associated with this residence.
