The effect of non-pharmacological intervention on brain plasticity in normal and pathological ageing

Abstract:

Stárnutí mozku je popisováno jako postupná ztráta neurofyziologických funkcí vedoucí k poklesu kognitivních funkcí. Strukturální změny, které jsou podkladem zhoršení kognice, zahrnují pokles v objemu šedé hmoty a změny v konektivitě mezi mozkovými sítěmi, tedy v bílé hmotě. U patologického stárnutí, např. u neurodegenerativních chorob, jsou tyto změny zřetelnější v oblastech mozku specifických pro danou nemoc. Při nízké efektivitě farmakoterapie těchto onemocnění se výzkum obrací na efekt různých forem nefarmakologických intervencí. Tanečně-pohybová intervence je komplexní aktivita kombinující fyzickou, kognitivní, sociální a uměleckou stimulaci. Cílem našeho výzkumu bylo zhodnotit efekt taneční intervence na mozkovou plasticitu – kognici a neurální koreláty, strukturální i funkční. Metody: zdraví senioři i pacienti s lehkou kognitivní poruchou (MCI) byli náhodně rozděleni do skupin – skupina taneční intervence (DI), která měla 60minutovou lekci tance tři krát týdně po dobu šesti měsíců, a kontrolní skupina, která vedla obvyklý život, bez větší fyzické aktivity (LAU). Všechny subjekty absolvovali vyšetření před a po intervenci – strukturální a funkční MRI, neuropsychologické vyšetření a vyšetření fyzické zdatnosti. V této dizertační práci je zhodnocen efekt taneční intervence na tloušťku kortexu, integritu bílé hmoty pomocí DTI a funkční konektivitu pomocí fMRI. Výsledky: v našem výzkumu jsme pozorovali několik signifikantních výsledků i pozitivních trendů. Kortikální tloušťka signifikantně narostla u DI skupiny v několika oblastech – inferiorní temporální, fusiformní a laterální okcipitální oblasti pravé hemisféry. Signifikantní efekt taneční intervence byl pozorován i v integritě bílé hmoty – bilaterálně ve fasciculus longitudinalis superior, v levém corpus callosum a bilaterálně v kortikospinálním traktu. U DI skupiny jsme pozorovali signifikantní nárust ve funkční konektivitě – v rámci sítě i mezi sítěmi – v porovnání s LAU skupinou, vyšší konektivita mezi pravou frontoparietální a salience sítí navíc korelovala se zlepšením v kognitivních testech zaměřených na pozornost a exekutivní funkce. Závěr: Náš výzkum ukázal, že 6měsíční tanečně-pohybová intervence má pozitivní vliv na kognitivní funkce a mozkovou plasticitu u zdravých seniorů i pacientů s MCI.

Abstract:

Background: Cerebral ageing can be described as a progressive loss of neurophysiological functions leading to cognitive decline. The structural changes underlying the deterioration include decrease in grey matter volume and changes in connectivity between brain networks, i.e. white matter changes. In pathological ageing, for example neurodegenerative diseases, these changes are more pronounced in disease-specific regions. With few or no effective treatment available for the diseases, the effect of various non-pharmacological interventions has been studied. Dance-movement intervention is a complex activity combining physical, cognitive, social and artistic stimulation. In this research, we aimed to assess the effect of the dance intervention on brain plasticity – cognitive outcome and underlying neural correlates, both structural and functional. Methods: Both healthy seniors and seniors with mild cognitive impairment (MCI) were randomly assigned into a dance-intervention group who underwent 6-month long dance-movement intervention (DI) taking place three times a week for 60 minutes, and a life as usual (LAU) group who led their normal lives, without extensive training. The subjects underwent examinations before and after the intervention, including structural MRI, resting state fMRI, neuropsychological examination and physical fitness examination. For this thesis we evaluated changes in cortical thickness, white matter integrity using DTI and functional connectivity using fMRI. Results: Several significant results as well as positive trends following the dance intervention were found in our research. Significant cortical thickening was found in inferior temporal, fusiform and lateral occipital regions of the right hemisphere in the dance-intervention group as well as increased radial diffusivity bilaterally in the superior longitudinal fasciculus, left corpus callosum and left corticospinal tract and increased mean diffusivity (P < .05) bilaterally in the superior longitudinal fasciculus, left corpus callosum and bilateral corticospinal tract. Significant fMRI results include increase in intra-network connectivity in default mode network in DI group compared to decrease in control group. There were six significant results in inter-network connectivity in DI group compared to control group. Moreover, the DI-induced changes in internetwork connectivity between the right frontoparietal and salience network significantly correlated with changes in attention domain and executive function domain z-scores. The significant cognitive outcomes were found in attention domain with increasing Z-score in DI group and decreasing in control group. Also, significant improvement was found in executive domain – in the five-point test in DI group following the intervention. Conclusions: Our research showed that supervised 6-month-long dance intervention induce positive effect on cognitive functions and distinct changes in brain plasticity as shown by both structural and functional MR imaging in both healthy seniors and seniors with MCI.
