Occupational therapy intervention for people after a stroke in the chronic stage

Abstract:

Cíl: Přispět k řešení problematiky ergoterapeutické intervence u osob po CMP v chronickém stádiu nemoci. Prohloubit poznatky o onemocnění a pomoci lépe zvládat všední denní činnosti osobám trpícím komplikacemi z následků CMP. Metodika: Práce je rozdělena na teoretickou a praktickou část. V teoretické části je popsána problematika CMP. Její celková charakteristika. Dále jsou popsána ergoterapeutická východiska při rehabilitaci osob s CMP. V praktické části je vypracována kazuistika klienta po CMP v chronickém stádiu nemoci. Informace byly se souhlasem klienta nasbírány z lékařské anamnézy, jež byla poskytnuta v zařízení Domov pro seniory Severní Terasa. Bylo využito metod pozorování, rozhovoru a testování. K vyšetření klienta byly použity následující testy: Barthelův test, FIM (Funkční míra nezávislosti), MMSE (Krátká škála mentálního stavu), WAB-R (Western aphasia battery - revised). Při terapii byly především používány techniky mobilizační, měkké techniky (míčkování), pasivního a aktivního provádění pohybů, rozvoje úchopů, nácviku kognitivních funkcí, rozvoje komunikace a nácviku ADL. Výsledky: Terapie nepřinesla signifikantní zlepšení zdravotního a psychického stavu klienta, avšak rovněž nebylo zaznamenáno žádné zhoršení stavu. Závěr: Zpracování problematiky CMP a aplikování získaných poznatků do průběhu terapie klienta přineslo další možné přístupy ergoterapeutické intervence u osob v chronickém stádiu CMP.

Abstract:

Goal: Improve solving problems with occupational intervension for people diagnosed with stroke in chronic stage of ilness. To deepen knowlegde about ilness and help klient with his activities of daily living. Methodology: Bacelor thesis is divided on teoretical and practical parts. The theoretical part consist of stroke disease issue. All round charasteristic. Also the occupational therapy basi sis pointed out while working with client. The practical part consist of casuistry of client after stroke disease in chronic stage. The information have been acuired after agreedment from client. Then the information have been colected from client's medical history, witch have been provided by Old people home Severní Terasa facility. The used methods have been watching, interview and testing. For client testing have been used this tests: Barthle index, FIM (Functional indepence measure), MMSE (Mini mental state examination), WAB-R (Western aphasia battery - revised). For thechniques have been used mobilization techniques, soft techniques (ball facilitation), active and passive movements, training of hand gripping, kognitive function education, komunication trainning and trainning of ADL. Results: The therapy did not show any significant improvment in physical and psychological state of client, never the less the state of client did not get any worse also. Conclusion: The utilization of occupation therapy in stroke disease and application of knowledge in practical therapy of the client showed new possibilities of occupational therapy interventions in chronic stage of stroke disease.
