Swallowing Disorders in Elderly Patients with Neurological Deficit

Abstract:

Úvod: Se zvyšujícím se věkem dochází k nárůstu prevalence poruchy polykání a to zejména u pacientů s CMP, degenerativními poruchami, zánětlivými a autoimunitními onemocněními a u dalších neurologických onemocnění. Cíl: Hlavním cílem této diplomové práce bylo zjistit výskyt poruch polykání u seniorů. Metodika: Jednalo se o kvantitativní výzkum, který zahrnoval systematické shromažďování a analýzu získaných informací. Byl využit standardizovaný nástroj (Screeningový test pro poruchy polykání), (Mandysová et al. 2012, s. 48) u 141 osob zahrnutých do výzkumného šetření. Data byla zpracována statisticky s využitím chí ? kvadrát testu, Wilcoxonova dvouvýběrového testu, Fisherova exaktního testu a také byl proveden výpočet senzitivity a specificity vybraného screeningového nástroje. Výsledky: V testovaném vzorku převládal počet pozitivních výsledků (67,4 %) screeningu, tedy převládaly osoby s možnou poruchou polykání. Nebyl nalezený statisticky významný rozdíl v zastoupení poruch polykání u jednotlivých diagnóz. V závislosti na kognitivních funkcích byl zjištěn významný statistický rozdíl ve výskytu rizika vzniku dysfagie. U osob se sníženými kognitivními funkcemi se vyskytovalo riziko vzniku dysfagie v 77 %. Podle analýzy zjištěných dat je zřejmé, že se stoupajícím věkem roste i podíl osob se zvýšeným rizikem vzniku poruchy polykání. Senzitivita zvoleného screeningového testu v porovnání s logopedickým vyšetřením je 100 %, kdy interval spolehlivosti byl 89,7?100,0 %. Specificita je 13 %, kdy interval spolehlivosti byl 2,8?33,6 %. Také byl zjištěn rozdíl v čase potřebném na provedení screeningového testu v závislosti na poruše polykání a také na kognitivních funkcích. Jak u pacientů s pozitivním výsledkem screeningového testu, tak u pacientů se sníženými kognitivními funkcemi byl potřebný delší čas na provedení daného testu. Závěr: Tento nástroj je díky své vysoké senzitivitě vhodný pro použití v klinické praxi k odhalování osob s rizikem vzniku poruchy polykání. Abstract:

Introduction: There is an increase in the prevalence of swallowing disorders with increasing age, especially in patients with stroke, degenerative disorders, inflammatory and autoimmune diseases and in other neurological diseases. Target: The main target of this dissertation was to find out the prevalence of swallowing disorders in the elderly. Methodology: It was about quantitative research that included the systematic collection and analysis of obtained information. It was used standardized tool (Screening test for swallowing disorders), (Mandysová et al. 2012, s. 48) by 141 people included in the survey. The data were processed statically using chi ? square test, Wilcoxson´s two-sample test and also it was made a calculation of sensitivity and specificity of selected screening tool. Result: Number of positive screening results (67,4 %) prevailed in the test sample which means that people with the potential swallowing disorders prevailed. There wasn´t found any statistically significant difference in representation of the swallowing disorders with different diagnosis. Depending on the cognitive functions it was observed significant statistical difference in the incidence of the risk of developing dysphagia. The risk of developing dyshagia was found in persons with impaired cognitive functions in 77 % of cases. According to the analysis of collected data, it is obvious that even the share of people with an increased risk of swallowing disorders increases with advancing age. The sensitivity of the chosen screening test in comparing with speech therapy examination is 100 %, when confidence interval was 89,7?100,0 %. Specificity is 13 %, when confidence interval was 2,8?33,6 %. It was also found a difference in the time that was required to make a screening test depending on swallowing disorder and also on cognitive functions. It was needed more time to complete the particular test as in patients with positive screening result so in patients with impaired cognitive functions. Conclusion: This tool is thanks to its high sensitivity suitable for using in clinical practise to detect persons at risk for developing swallowing disorders.
