The experience of nurses caring for patients with delirium at intensive care units

Abstract:

Úvod: Delirium je spojováno s hyperaktivními projevy pacientů, kdy mnohdy slovně a fyzicky napadají ošetřující personál. To způsobuje negativní přístup sester k poskytování ošetřovatelské péče, která je však klíčová pro prevenci vzniku komplikací spojených s deliriem. Cíl: Cílem diplomové práce bylo zjistit zkušenosti a vnímání sester poskytujících ošetřovatelskou péči pacientům s deliriem na jednotkách intenzivní péče. Metodika: Kvalitativní data byla zjišťována formou polostrukturovaných rozhovorů a následně pak zpracována pomocí tematické analýzy. Výzkumného šetření se zúčastnilo celkem 12 sester pracujících na jednotkách intenzivní péče (dále jen JIP). Výsledky: Identifikovali jsme 3 hlavní témata: diagnóza delirium, zkušenosti sester s péčí o pacienty s deliriem na JIP a systém péče. Z rozhovorů s respondenty vyplynulo, že disponují neadekvátními znalostmi o deliriu. Někteří uvedli, že přítomnost deliria hodnotí dle svých osobních zkušeností. Péči o pacienty s deliriem většina respondentů popisovala jako psychicky a fyzicky náročnou, přičemž považovali za nejúčinnější intervenci ke zvládnutí agitovaných projevů pacientů fyzické a farmakologické omezení. Někteří respondenti vnímali bariéry zejména v nedostatečném nastavení systému poskytované péče, přičemž zároveň zmínili potřebu navýšení kompetencí sester ve fyzickém omezení a podávání léčiv. Závěr: Nedostatečné zkušenosti sester s péčí o pacienty s deliriem na JIP ztěžují poskytování kvalitní péče a zvyšují její náročnost, což může vést k syndromu vyhoření a následné fluktuaci všeobecných sester. Doporučením pro klinickou praxi v péči o pacienty s deliriem je zvýšit informovanost ošetřujícího personálu týkající se problematiky deliria a realizovat další výzkumná šetření týkající se jak zkušeností sester, tak pacientů s deliriem hospitalizovaných na JIP.

Abstract:

Introduction: Delirium is associated with hyperactive symptoms of patients, who often verbally and physically attack the nursing staff. This causes a negative attitude of nurses in providing nursing care, which is however crucial in preventing complications associated with delirium. Aim: The aim of this thesis was to find out the experience and perceptions of nurses providing nursing care to patients with delirium in intensive care units. Methodology. Qualitative data was collected by semi-structured interviews and then processed using thematic analysis. A total of 12 nurses working in intensive care units (ICUs) participated in the research. Results. We identified 3 main topics: diagnosis of delirium, nurses' experience of caring for patients with delirium in the ICU and the care system. Interviews with respondents revealed that they had inadequate knowledge about delirium. Some stated that they evaluate the presence of delirium based on their personal experience. The care of delirium patients was described as mentally and physically demanding, and physical and pharmacological restraints were perceived as the most effective interventions to manage patients' agitated symptoms. Some respondents perceived barriers mainly in the inadequate setup of the care delivery system, while at the same time mentioning the need to increase nurses' competencies in physical restraint and medication administration. Conclusion: Lack of experience of nurses in caring for delirium patients in the ICU makes it difficult to provide quality care and increases the complexity of care, which may lead to burnout syndrome and turnover of general nurses. Recommendations for clinical practice in the care of patients with delirium would be increasing nursing staff awareness of delirium and conducting further research investigations on both nurses' and ICU delirium patients' experience with delirium.
