The Incidence of postoperative nausea and vomiting (PONV) in children undergoing endoscopic adenoidectomy under general anesthesia and its prevention.

Abstract:

Pooperační nauzea a zvracení (PONV) je velmi častá pooperační komplikace, kterou pacienti subjektivně hodnotí hůře než pooperační bolest. PONV s sebou nese řadu nežádoucích následků, jako například prodloužení hospitalizace či neočekávané opětovné přijetí do nemocnice u ambulantně prováděných výkonů. Silné zvracení může také způsobit dehiscenci operační rány, rupturu jícnu, aspiraci, dehydrataci s metabolickým rozvratem, atd. Cílem diplomové práce bylo zjistit, jaký je skutečný výskyt PONV u dětí po provedení endoskopické adenotomie v celkové anestezii, tyto výsledky porovnat s hodnoticími škálami ke stratifikaci rizika vzniku PONV a zhodnotit tak jejich validitu pro daný operační výkon. Dalším cílem bylo zhodnocení vlivu kinetózy a expozice cigaretovému kouři na výskyt PONV. Posledním cílem bylo prostřednictvím dotazníkového šetření na vybraných klinických pracovištích (Ostrava, Brno, Praha) zjistit reálný přístup k prevenci PONV po endoskopické adenotomii. Výzkumný vzorek tvořilo 200 dětských pacientů, kteří tento výkon podstoupili v období leden 2020 - září 2020 a 27 lékařů (anesteziologů) z FNO (Fakultní nemocnice Ostrava), 26 lékařů z KDAR FN Brno (Klinika dětské anesteziologie a resuscitace Fakultní nemocnice Brno) a 25 anesteziologů z KARIM FN Motol (Klinika anesteziologie, resuscitace a intenzivní medicíny Fakultní nemocnice Motol). Data byla získána kvantitativním výzkumem pomocí dotazníkového šetření a retrospektivní analýzou zdravotnické dokumentace s následným statistickým zpracováním výstupů. Jednotlivé vztahy mezi získanými parametry byly vyhodnoceny pomocí Pearsonova chí - kvadrátového testu dobré shody a Pearsonovým chí - kvadrátovým testem nezávislosti. Statistické testy byly hodnoceny na hladině významnosti 5 %. Ze získaných výsledků vyplynulo, že endoskopická adenotomie je výkon se středním rizikem vzniku PONV, s nízkým rizikem vzniku PDNV (postdischarge nausea and vomiting). Přítomnost kinetózy a expozice cigaretovému kouři vede ke zvýšení incidence PONV. Na základě podrobného prozkoumání problematiky a zjištěných dat, lze konstatovat, že jde o aktuální problematiku. Ošetřovatelský personál a rodiče dětí představují nedílnou součástí pro adekvátní a úspěšnou pooperační péči.

Abstract:

Postoperative nausea and vomiting (PONV) is a very common postoperative complication, which patients subjectively assess worse than postoperative pain. PONV has a number of adverse consequences, such as prolonged hospitalization or unexpected re-admission to the hospital for outpatient services. Severe vomiting can also cause surgical wound dehiscence, esophageal rupture, aspiration, dehydration with metabolic disruption, etc. The aim of the thesis was to determine the true incidence of PONV in children after endoscopic adenotomy under general anesthesia, to compare these results with evaluation scales for stratification risks of PONV and evaluate their validity for the given operational performance. Another goal was to evaluate the effect of kinetosis and exposure to cigarette smoke on the incidence of PONV. The last goal was to find out a real approach to the prevention of PONV after endoscopic adenotomy through a questionnaire survey at selected clinical sites (Ostrava, Brno, Prague). The research sample consisted of 200 pediatric patients who underwent this procedure in the period January 2020 - September 2020 and 27 physicians (anesthesiologists) from FNO (University Hospital Ostrava), 26 physicians from KDAR FN Brno (Department of Pediatric Anesthesiology and Resuscitation, University Hospital Brno) and 25 anesthesiologists from KARIM FN Motol (Department of Anesthesiology, Resuscitation and Intensive Care, Motol University Hospital). The data were obtained by quantitative research using a questionnaire survey and retrospective analysis of medical documentation with subsequent statistical processing of outputs. The individual relationships between the obtained parameters were evaluated using Pearson 's chi - square test of good agreement and Pearson' s chi - square test of independence. Statistical tests were evaluated at a significance level of 5%. The obtained results showed that endoscopic adenotomy is a procedure with a medium risk of developing PONV, with a low risk of developing PDNV (postdischarge nausea and vomiting). The presence of motion sickness and exposure to cigarette smoke leads to an increase in the incidence of PONV. Based on a detailed examination of the issue and the data obtained, it can be stated that this is a current issue. Nursing staff and children's parents are an integral part of adequate and successful postoperative care.
