Elderly attitudes to death, dying and palliative care

Abstract:

Cíl: Cílem diplomové práce bylo zhodnotit postoje seniorů ke smrti, umírání a paliativní péči. Metodika: Výzkum má charakter kvantitativního šetření. Ke sběru dat byl použit dotazník. Do výzkumného šetření bylo zahrnuto 154 seniorů žijících v domácím prostředí. K hodnocení postojů seniorů ke smrti, umírání a paliativní péči byl použit dotazník ?Older adults´attitudes to death, palliative treatment and hospice care?. Součástí dotazníku byly škály: GDS (Geriatrická škála deprese) pro zhodnocení deprese a Death Anxiety Scale (DAS ? Škála úzkosti ze smrti) ? pro zhodnocení úzkosti ze smrti. Výzkum byl realizován od srpna roku 2015 do listopadu 2015. Výsledky: U 118 (tj. 77 %) seniorů nebyla potvrzena deprese. Mírná deprese byla potvrzena u 28 (tj. 18 %) seniorů a 8 (tj. 5 %) seniorů vykazovalo manifestní depresi. Průměrná hodnota škály úzkosti ze smrti byla zjištěna 22,2 bodů na stupnici 8?40 bodů, což svědčí o vyšší míře úzkosti ze smrti. Byl zjištěn statisticky významný rozdíl v postojích seniorů k eutanázii dle věku (p = 0,0213), v pohledu seniorů na umírání dle věku (p = 0,0441), v postojích seniorů k hospicové péči dle stupně vzdělání (p = 0,0001) a v postojích seniorů k hospicové péči dle způsobu bydlení (p = 0,0056). Statisticky významný rozdíl byl dále zjištěn v postojích seniorů ke kontrole bolesti dle přítomnosti deprese (p = 0,0182). Závěr: Postoje seniorů k eutanázii a pohledu na umírání se lišily dle věku. Postoje k hospicové péči se lišily dle stupně vzdělání a způsobu bydlení. Postoje seniorů ke kontrole bolesti se lišily dle přítomnosti deprese.

Abstract:

Objective: The aim of the thesis was to evaluate seniors' attitudes towards death, dying and palliative care. Methodology: The nature of research is a quantitative survey. Data were collected by the means of a questionnaire. The research survey included 154 seniors living in at-home setting. To evaluate the attitudes of seniors towards death, dying and palliative care, the questionnaire called "Older adults' attitudes to death, palliative treatment and hospice care" was used. The questionnaire included GDS scale (Geriatric Depression Scale) for evaluation of depression, and Death Anxiety Scale (DAS) to evaluate the anxiety of death. The research was conducted from August 2015 to November 2015. Results: In 118 elderly (i.e. 77 %) depression was not confirmed. Mild depression was confirmed in 28 elderly (i.e. 18 %), and 8 elderly (i.e. 5 %) showed manifest depression. The average value of the death anxiety scale was found to be 22,2 points on the scale from 8 to 40 points, indicating a higher level of anxiety of death. Statistically significant difference was discovered in the attitudes of elderly to euthanasia in individual age groups (p = 0,0213), in the views of older people on dying in individual age groups (p = 0,0441), in the attitudes of older people to hospice care in different education levels (p = 0,0001) and in the attitudes of older people to hospice care in various types of housing (p = 0,0056). A statistically significant difference was also observed in the attitudes of seniors towards the control of pain according to the presence of depression (p = 0,0182). Conclusion: The elderly attitudes towards euthanasia and their view on death differed according to their age. Attitudes towards the hospice care varied according to the level of education and the type of housing. Attitudes of elderly towards the control of pain varied according to the presence of depression.
