The Rehabilitation of Post-Stroke Patients: A Combination of Physiotherapy and Occupational Therapy in Patients Over the Age of 75

Abstract:

Tato diplomová práce se zabývá problematikou rehabilitace u geriatrických pacientů ve věku nad 75 let s diagnózou cévní mozkové příhody na doléčovacích lůžkách. Cílem práce bylo posoudit vliv kombinované rehabilitační léčby na zlepšení míry funkční nezávislosti a samostatnosti, posouzení vlivu ergoterapie na zlepšení míry funkčních schopností a zhodnocení vlivu rehabilitační intervence v jednotlivých činnostech běžného života (ADL) Daná studie byla provedena na Doléčovacím a rehabilitačním zařízení Fakultní nemocnice u Svaté Anny v Brně. Do studie bylo celkem zařazeno 57 pacientů s průměrným věkem 84,5 let (SD ± 5,9), 38 žen a 19 mužů. Pro posouzení vlivu ergoterapie byl daný soubor dále rozdělen na skupinu 39 pacientů (průměrný věk 83,3 let, SD ± 3,8), který měl do svého rehabilitačního programu zařazenou jen fyzioterapii a na skupinu 18 pacientů (průměrný věk 83,3 let, SD ± 3,8), která měla v rámci komplexní rehabilitace zařazenou fyzioterapii i ergoterapii. Jako hodnotící nástroj byl zvolen Test funkční nezávislosti (FIM test). Vstupní vyšetření bylo provedeno před zahájením rehabilitační léčby a výstupní vyšetření bylo odebráno v den propuštění domů nebo do zařízení následné péče. Ke statistickému zhodnocení byl použit Wilcoxonův párový test. Z výsledků vyplynulo, že komplexní rehabilitace u geriatrických pacientů po CMP vede ke zvýšení míry funkční nezávislosti a zlepšuje jejich soběstačnost v základních denních činostech. Tím je potvrzena nutnost následné komplexní péče u těchto pacientů. Výsledky práce také dokazují, že ergoterapie zaměřená na nácvik běžných denních činností výrazně přispívá ke zvýšení pacientovi soběstačnosti a tím zlepšuje jeho kvalitu života.

Abstract:

This thesis deals with issues surrounding the rehabilitation of post-stroke geriatric patients, in the age category of 75 years and older. The goal of this research was to determine the influence of combined rehabilitation treatment on the improvement of patients‘ independence and self-sufficiency, to ascertain the effect of occupational therapy on improvements in various functional abilities and to evaluate the effect of rehabilitation intervention on individual activities performed in daily life (ADL). This study was conducted at the rehabilitation centre at St. Anne Faculty hospital in Brno. It included a total of 57 patients, 38 women and 19 men, whose average age was 84.5 years (SD  5.9). In order to determine the effect that occupational therapy had on the patients, the sample was further subdivided into a group which included 39 patients, whose average age was 83.3 years (SD  3.8) and whose rehabilitation plan consisted only of physiotherapy. The other sample group was made up 18 patients, averaging 83.3 years in age (SD  3.8), whose comprehensive rehabilitation plan included both physiotherapy as well as occupational therapy. The assessment tool that was chosen for this study was the test of functional independence (FIM). The initial examination was carried out before the onset of the patients‘ rehabilitation treatment and the final check up was conducted on the day that the patients were released to either go home or to continue further treatment in another institution. The Wilcoxon matched pair test was used in this statistical analysis. The results show that undergoing comprehensive rehabilitation leads to greater levels of functional independence and to improvements in self-sufficiency as well as basic daily activities in post-stroke geriatric patients. The results further prove that occupational therapy that is focused on the training of common daily activities contributes markedly to patients‘ greater self-sufficiency, and thereby improves their quality of life.
