Fear and pain in children with a fracture

Abstract:

ABSTRAKT Cíl: Cílem práce je zhodnotit strach a intenzitu bolesti před operačním zákrokem u dětí se zlomeninou končetin přijatých na dětská oddělení. Metodika: Výzkumného šetření se zúčastnilo 166 respondentů. Výzkumný soubor tvořily děti ve věku 6-15 let, které splňovaly určená kritéria. Dotazníkové šetření probíhalo se souhlasem náměstkyně pro ošetřovatelskou péči na dětských lůžkových odděleních nemocnic. Výzkum se uskutečnil v období leden-březen 2021. K výzkumu byla použita česká verze standardizovaného dotazníku strachu z operace (Surgical Fear Questionnaire) z roku 2016. Autorem dotazníku této verze je Renata Zeleníková, PhDr., PhD. Dotazník byl doplněn o demografické údaje a škály hodnocení intenzity bolesti - Vizuální analogová škála (VAS) a Present Pain Intensity (PPI). Výsledky: Hodnocení výzkumných údajů získaných z dotazníku SFQ a souběžně použitého dokumentu demografické údaje dokládá, že v okamžiku před operací existuje velmi silná závislost zkoumané veličiny intenzita bolesti na velikosti strachu z vlastní operace. Zpracování vstupních dat o vnímané intenzitě bolesti, včetně jejího časového průběhu, spojené se zkoumáním vlivu ostatních veličin obsažených ve výzkumu, věk pacienta, pohlaví, lokalizace zlomeniny a zvolený léčebný postup poranění ukazuje, že z uvedených veličin má na intenzitu bolesti a její časový průběh statisticky významný vliv pouze zvolený léčebný postup. Ostatní sledované veličiny s velkou pravděpodobnosti mají na intenzitu bolesti malý vliv. Závěr: Diplomová práce představuje některé neobvyklé pohledy na pojmy, se kterými se sice v nemocniční praxi každodenně setkáváme, přesto však mnohdy zůstávají v průběhu ošetřovatelské péče o děti velkou neznámou.

Abstract:

ABSTRACT Aim: The aim of this thesis is to assess the fear and the pain intensity before a surgery in children with a fractured limb admitted to a Pediatric Unit. Methodology: In total 166 respondents have taken part in the research survey. The research sample consists of children aged 6-15 who fulfill the pre-defined criteria. The research survey took place with the consent of the Deputy for Nursing Care at Hospital°s' Pediatric Units. The research took place between January and March 2021. A Czech version of the standardized questionnaire on surgical fear (SurgicalFearQuestionnaire) from 2016 was used in the research. The author of this version is Renata Zeleníková, PhDr., PhD. The questionnaire was complemented with demographical data and pain intensity assessment scales - the Visual Analogue Scale (VAS) and the Present Pain Intensity (PPI). Results: The analysis of the research data obtained from the SFQ questionnaire and from the simultaneously used document on demographical data shows that at the moment before the surgery there is a very strong dependency of the assessed pain intensity on the fear intensity from the surgery itself. The processing of the entry data on perceived pain intensity, including its course over time, together with assessing the influence of the other factors included in the research such as patient's age, gender, the place of the fracture and the chosen injury treatment procedure shows that out of the aforementioned factors it is only the chosen treatment procedure that has a statistically significant impact on the pain intensity and its course over time. The other monitored factors have with a high degree of probability low impact on the pain intensity. Conclusion: This diploma thesis presents a few unusual views on concepts that we experience in hospital practice every day and that, however, often remain unfamiliar during the children treatment care procedure.
