Nursing care of patients with spinal cord injury

Abstract:

Tématem bakalářské práce je ošetřovatelská péče o pacienty s míšním poraněním. Tato práce je rozdělena na část teoretickou a obecným plánem péče. Teoretická část se zabývá patofyziologii, etiologii, diagnostikou a léčbou onemocnění. Obecný plán v části praktické se zabývá ošetřovatelskou péči o spinálního pacienta - od příjmu pacienta do nemocnice po jeho propuštění. Jsou stanoveny ošetřovatelské diagnózy dle Nanda International Taxonomie II 2012 ? 2014. Cílem této práce je popis ošetřovatelské péče o pacienta se spinálním poraněním, od jeho příjmu na specializované pracoviště, jeho zajištění, diagnostika, předoperační a pooperační péče, ve které mnohdy nastávají komplikace. Probíhá intenzivní rehabilitace a celkové vyrovnávání se s handicapem, které bývá ?bojem? nejtěžším. Spinální poranění, jehož následkem je ochrnutí zasáhne do života člověka ve všech oblastech. V závislosti na typu a rozsahu ochrnutí dochází ke změně nejen tělesného, ale i psychického stavu, který velmi často bývá špatný. Hospitalizace těchto pacientů bývá zdlouhavá a probíhá na specializovaných pracovištích, nejlépe však na spinálních jednotkách, kde se o něho stará celý tým zdravotnických pracovníků složený z lékařů - neurochirurgů, všeobecných sester, v některých případech i psychologů a fyzioterapeutů, kteří mají svoji nezastupitelnou roli v intenzivní rehabilitaci. Péče bývá velmi náročná po stránce fyzické i psychické a je zapotřebí, aby byla prováděna co nejzodpovědněji a nejpečlivěji z důvodů prevence vzniků mnoha dalších komplikací, které mohou nastat.

Abstract:

The subjekt of this bachelor's dissertation is nursing care for patiens with spinal cord injury. This dissertation is consisting of two parts: theoretical and general nursing care guidelines. Theoretical part consists of pathophysiology, etiology, diagnostics and treatment. The general nursing guideline portion consists of practical nursing care guidelines from admitting to releasing the patient from the hospital. The nursing diagnosis are in accordance with NANDA International Nursing Diagnosis definition and classification for 2012 ? 2014. The objektive of this dissertation is to present nursing care practices starting with the patient's admission to the specialized unit, diagnosis, pre and post - op care and other therapeutic processes that oftentimes come with complications. The intensive physical therapy and coming to terms with a handicap is the hardest battle the patiens with spinal cord injury face. Spinal cord injury with a resulting paralysis changes all aspects of patent's life. Depending on the type and the extent of the injury there are not only physical but oftentimes negative psychological changes in patients. The patients are hospitalized for extended periods of time in specialized treatment units, preferably at the Spinal Injuries Unit, where they are cared for by a team of specialists including neurosurgeons, nurses and in some cases psychologists and physical therapists that have a crucial role in intensive rehabilitation. The care is challenging physically and psychologically and it is important that it is provided responsibly and diligently so any possible complications are prevented.
