Nursing process in Care of Pregnant woman with HELLP syndrome

Abstract:

Bakalářská práce řeší problematiku syndromu HELLP. Tento stav popisujeme jako vážnou život ohrožující těhotenskou komplikaci, která je sice vzácná, ale v porodnictví stále aktuálně řešené téma. Bakalářská práce je rozdělena na část teoretickou a část praktickou. Cílem této práce je vypracování ošetřovatelského procesu, který by mohl mít využití porodními asistentkami v praxi v případě žen s touto komplikací. Teoretická část se zabývá podstatou onemocnění, tedy definicí, klasifikačními kritérii, vznikem, průběhem, diagnostikou a léčbou onemocnění. V této části práce je popsán postup v péči o těhotnou ženu s HELLP syndromem z pohledu porodní asistentky a na závěr jsou vypsány veškeré možné komplikace postihující ženu s diagnózou HELLP. V praktické části je popsána celková anamnéza klientky společně s celkovým průběhem hospitalizace na lůžkovém oddělení šestinedělí gynekologicko-porodnické kliniky Fakultní nemocnice Ostrava. Na základě získaných informací z dostupné zdravotnické dokumentace, dostupných laboratorních vyšetření a nálezů společně s informacemi od samotné klientky a zdravotnického personálu, byl sestaven ošetřovatelský proces se všemi náležitostmi zpracovaný za pomoci posouzení klientky v jednotlivých doménách NANDA International. Dle tohoto posouzení byly stanoveny ošetřovatelské diagnózy dle NANDA I. taxonomie II vztahující se na konkrétní potřeby klientky s HELLP syndromem. Součástí ošetřovatelského procesu bylo stanovení ošetřovatelských cílů, intervencí, realizace a závěrečného hodnocení.

Abstract:

Bachelor thesis deals with the issue of HELLP syndrome. We describe this condition as a serious life-threatening pregnancy complication, which is a rare but still currently solved topic in obstetrics. The bachelor thesis is divided into theoretical and practical part. The aim of this thesis is to create a nursing process that could be used by other midwives in practice for women with this complication. The theoretical part deals with the principle of the disease, the definition of the disease, classification criteria, development, disease progression, diagnosis and treatment of the disease. In this part of the thesis is described the procedure in the care of a pregnant woman with HELLP syndrome from the point of view of a midwife and at the end of this part are possible complications affecting a woman diagnosed with HELLP syndrome. The practical part describes the general history of the patient together with the overall course of hospitalization at the gynecological-obstetric department of the University Hospital in Ostrava. Based on the information obtained from available medical documentation, available laboratory tests and findings together with information from the patient and midwives itself, a nursing process was prepared with all the requisites using the patient's assessment in individual NANDA International domains. Based on this assessment, nursing diagnoses were established by NANDA I. taxonomy II. related to the specific needs of the client with HELLP syndrome. The part of the nursing process was to determine nursing aims, interventions, realization and final evaluation.
