Community midwifery care in England and the Czech Republic

Abstract:

Cílem diplomové práce je specifikovat případné rozdíly, vyskytující se v systému komunitní péče porodních asistentek, působících na území Anglie a České republiky. Konkrétně se výzkum zaměřuje na zjištění případných rozdílů v profesních kompetencích, v systému financování, v potřebách péče, službách nabízených klientkám i roli, kterou porodní asistentky zastávají v rámci komunity v jednotlivých zemích. Kvalitativní výzkumné šetření bylo provedeno prostřednictvím polostrukturovaných rozhovorů, které byly vedeny se třemi českými a třemi anglickými porodními asistentkami, pracujícími v komunitní péči. Byly stanoveny celkem tři hlavní kategorie a jedenáct subkategorií, charakterizující role komunitních porodních asistentek, odbornou činnost, platební podmínky i obecná specifika systému komunitní péče. Z analýzy výsledků je patrné, že komunitní péče porodních asistentek v Anglii je na rozdíl od České republiky zcela běžnou a poměrně žádanou formou poskytované zdravotnické péče. Je zde adekvátně podporována kontinuita role porodních asistentek v péči o ženu a dítě i přání žen, které odmítají lékařskou péči. Porodní asistentky mají jasně danou pracovní dobu, během níž pracují s klientkami v závislosti na jejich individuálních potřebách. U žen zajišťují péči od počátku těhotenství až do období šestinedělí. Služby komunitních porodních asistentek, pracujících v zařízeních Národního zdravotnického systému, jsou placeny z veřejného zdravotního pojištění. Zatímco v České republice je potřeba pomoci porodních asistentek ženám v komunitním prostředí neustále zpochybňována, v Anglii je považována za jeden z nejvhodnějších a nejbezpečnějších způsobů, jak prožít období těhotenství, porodu i šestinedělí.

Abstract:

The goal of the dissertation is to specify any differences occurring in the system of community care midwives working in England and the Czech Republic. Specifically, the research is focused on finding out the differences in professional competences, funding system, care needs, services offered to the clients and also in the role of midwives in the community in this countries. The qualitative research was conducted through semi-structured interviews, which were accomplished with three Czech and three English midwives working in the community care. There were set three main categories and eleven subcategories describing the roles of community midwives, professional activity, payment terms and also the general specifics of the system in the community care. The analysis of the results show, that the community of the midwifery care in England, unlike the Czech Republic, is completely prevalent and quite requested from the healthcare delivery. There is adequate support of the continuity for the midwives roles in the care of a wife and child and also of the wishes of women, who refuse the medical care. The midwives have clearly given working period, during which they are working with clients depending on their individual needs. They are providing the necessary care from early of pregnancy to the postpartum period. The services of community midwives, who are working in the facilitiesof the National Health System, are paid from the public health insurance. While in the Czech Republic is the effort to help women by midwives in community settings still continually in question, in England is considered the best and safest ways to experience periods of pregnancy, childbirth and the postpartum period.
