Use of occupational therapy in the training of communication skills with person with cognitively communication deficit

Abstract:

Cíl: Cílem bakalářské práce je přiblížit úlohu ergoterapeuta a poukázat na ergoterapeutické postupy u klientky s diagnózou afázie následkem cévní mozkové příhody při nácviku komunikační dovednosti. Metodika: Práce je rozdělena na teoretickou a praktickou část. Z názvu vyplývá, že je práce zaměřena na nácvik komunikace u osoby s kognitivně - komunikačním deficitem. V teoretické části je obecný popis komunikace, motorických a kognitivních funkcí a jejich poruchy, afázie a diagnostické postupy. Dále popsání logopedické dostupnosti a ergoterapie u afázie. Praktická část je zaměřena na kazuistiku klientky po cévní mozkové příhodě s afázií ve starším věku, zpracování její anamnézy, provedení vstupního a následně výstupního hodnocení. Při vstupním hodnocení se použil: rozhovor s klientkou, Barthelův test, Montreálský kognitivní test MoCA - CZ3, dotazník funkcionální komunikace, The Mississippi Aphasia Screening Test (MASTcz) a škála deprese dle Yesavage (GDS - 15). Při výstupním hodnocení se kromě Barthelova testu, z důvodu žádných změn v oblasti ADL, provedlo vše. Poté byl sestaven doporučený dlouhodobý ergoterapeutický plán. Výsledky: Díky zvoleným metodám a postupům se odboural stres z komunikace a došlo k mírnému zlepšení ve většině oblastech v rámci ergoterapeutické intervence. Závěr: Tato bakalářská práce mi pomohla rozšířit si obzory v této problematice a možnostech ergoterapeutické intervence. Všechny cíle, které byly nastavené až na pár výjimek se podařilo splnit.

Abstract:

Purpose: The purpose of the bachelor thesis is to approach the role of an occupational therapist and to point out occupational therapy procedures for a client with a diagnosis of aphasia as a result of a stroke during communication skills training. Methodology: The work is divided into theoretical and practical part. As the name implies the work is focused on practicing communication in a person with a cognitive - communication deficit. The theoretical part is a general description of communication, motor and cognitive functions and their disorders, aphasia and diagnostic procedures. Further description of speech therapy availability and occupational therapy in aphasia. The practical part is focused on the client's case report after a stroke with aphasia in old age processing of it´s anamnesis performing the initial and subsequent final examination. For the examination was used to: an interview with a client, Barthel test, the Montreal Cognitive Test MoCA - CZ3, a functional communication questionnaire, The Mississippi Aphasia Screening Test (MASTcz) and the Yesavage Depression Scale (GDS - 15). During the final examination everything was done except Barthel test due to no changes in the area of ADL. A recommended long-term occupational therapy plan was then established. Results: Thanks to the chosen methods and procedures, the stress from communication was eliminated and there was a slight improvement in most areas within the occupational therapy intervention. Conclusion: This bachelor thesis helped me to broaden my horizons in this area and the possibilities of occupational therapy intervention. All the goals, which were set with a few exceptions were met.
