Nursing case report a patient with poisoning Metformin

Abstract:

Bakalářská práce je zaměřena na ošetřovatelskou péči o pacientku s otravou Metforminem. Popisuje problematiku lékových intoxikací především otravu Metforminem s následnou laktátovou acidózou. K jejímu zpracování jsem zvolila metodu kazuistiky, jelikož je pro jeho prezentaci nejvhodnější. Hlavním cílem bakalářské práce bylo podrobně popsat ošetřovatelskou péči u pacientky hospitalizované na jednotce intenzivní péče s otravou Metforminem a následnou laktátovou acidózou a vytvořit tak návrh ošetřovatelského postupu pro péči o pacienty s touto diagnózou. Ošetřovatelská péče byla poskytována metodou ošetřovatelského procesu. Zpracování ošetřovatelské anamnézy bylo provedeno podle modelu funkčního zdraví Marjory Gordonové. Na základě zjištěných informací byly stanoveny ošetřovatelské problémy podle NANDA-I taxonomie II a sestaven ošetřovatelský plán. Z aktuálních diagnóz byla nejzávažnější zhoršená spontánní ventilace, deficit sebepéče při stravování, koupání a vyprazdňování. Z potenciálních diagnóz pak riziko dekubitu a riziko infekce. Na stanovené ošetřovatelské diagnózy byla zaměřena následná ošetřovatelská péče a naplánovanými intervencemi jsme se snažili docílit prevence, odstranění nebo alespoň zmírnění problémů. Cíle a výsledná kritéria se nám podařilo ve větší míře splnit a docílili jsme tak výrazného zlepšení zdravotního stavu pacientky. Práce potvrzuje, že kvalitně poskytovaná ošetřovatelská péče je nepostradatelná v komplexní péči o pacienty, a že je stejně důležitá, jako péče lékařská. Specifika ošetřovatelské péče u pacientky s intoxikací metforminem se výrazně neliší od péče o kriticky nemocné pacienty. Může být proto návodem pro sestry pracující na jednotce intenzivní péče, které pečují o kriticky nemocné pacienty a také studijním materiálem pro studenty nelékařských zdravotnických oborů.

Abstract:

Bachelor thesis is focused on the nursing care of the patient with poisoning Metformin. Describes the issue of drug-drug intoxication mainly poisoning Metformin with subsequent lactic acidosis. For its processing, I chose the method of case report, as it is for its presentation of the most suitable. The main aim of this thesis was to describe in detail the nursing care for patients hospitalized in the intensive care unit with poisoning Metformin and subsequent lactic acidosis, and create a proposal for a nursing procedure for care of patients with this diagnosis. Nursing care was provided by the method of the nursing process. The processing of the nursing anamnesis was carried out according to the model of functional health Marjory Gordon. On the basis of the detected information are set nursing problems according to NANDA-I taxonomy II and the assembled care plan. Of the current diagnoses were the most serious impaired spontaneous ventilation, the deficit of self-care in eating, bathing, and defecation. Of potential diagnoses, then the risk of decubitus wound and risk of infection. On established nursing diagnoses were focused subsequent nursing care and planned interventions, we tried to achieve the prevention, elimination or at least mitigate the problems. The objectives and resulting criteria we have succeeded to a greater extent fulfil and we achieved such a significant improvement in the health status of the patient. The work confirms that the quality of the provided nursing care is indispensable in the comprehensive care of patients, and that is just as important as care medical. Specifics of nursing care for patients with intoxication metformin is not significantly different from the care of critically ill patients. It can therefore be guidance for the nurses working in the intensive care unit, caring for critically ill patients and also study material for the students of paramedical fields of study.
