Cortico-subcortical interactions in epilepsy

Abstract:

Hlavním tématem mé disertační práce jsou kortiko-subkortikální interakce u epilepsie, konkrétně hluboká mozková stimulace (DBS) u pacientů s farmakorezistentní epilepsií. DBS je moderní léčebná metoda založená na elektrické stimulaci hlubokých mozkových struktur s využitím elektrod implantovaných do hlubokých mozkových struktur – v tomto případě se jedná o přední jádra thalamu nebo mediální pulvinar thalamu. Můj první článek Thalamic oscillatory activity may predict response to deep brain stimulation of the anterior nuclei of the thalamus (Deutschová B et al, Epilepsia, 2021) se věnoval predikci outcomu pacientů s farmakorezistentní epilepsií a hlubokou mozkovou stimulací. Elektrofyziologické vlastnosti předních jader thalamu zaznamenané po implantaci hlubokých mozkových elektrod můžou hrát významnou roli v predikci účinnosti DBS terapie. Pro budoucnost by byla hlavním cílem analýza prediktivních biomarkerů před implantací hlubokých mozkových elektrod. Můj druhý článek Ictal connectivity changes induced by pulvinar stimulation correlate with improvement of awareness (Deutschová B et al, Brain Stimul, 2021) vznikl díky mé odborné stáži ve Francii a spolupráci s profesorem Fabrice Bartolomei z výzkumného pracoviště Service de Neurophysiologie Clinique, Institut de Neurosciences des Systèmes (INS), INSERM. V této studii jsme se zaměřili na neuronální mechanismy, které jsou podkladem alterace vědomí (AOA) a na účinky stimulace pulvinaru. Bylo prokázáno, že stimulace mediálního pulvinaru je efektivní metodou k zmírnění alterace vědomí. Jsem také spoluautorkou článku Resting-state phase-amplitude coupling between the human subthalamic nucleus and cortical activity: a simultaneous intracranial and scalp EEG study (Damborská A et al, Brain Topogr, 2021). Naše výsledky ukazují, že nucleus subthalamicus (STN) je významnou strukturou kortiko-subkortikálních okruhů lidského mozku a jednou z důležitých otázek bylo, zdali je unilaterální DBS stimulace STN dostačující pro léčbu Parkinsonovy nemoci.

Abstract:

My thesis is devoted to cortico-subcortical interactions in epilepsy, specifically to deep brain stimulation (DBS) in patients with pharmacoresistant epilepsy. DBS is a modern therapeutic method based on chronic electrical stimulation of brain structures via electrodes implanted in deep brain structures – for example, in the anterior nuclei of the thalamus (ANT), or in the medial pulvinar of the thalamus (PuM). My first article Thalamic oscillatory activity may predict response to deep brain stimulation of the anterior nuclei of the thalamus (Deutschová B et al, Epilepsia, 2021) relates to the patients with pharmacoresistant epilepsy. We proposed that local EEG characteristics of ANT recorded after implantation may play a significant role in DBS response prediction. In the foreseeable future, the aim will be to analyze predictive biomarkers prior to the implantation of the deep brain electrodes. My second article Ictal connectivity changes induced by pulvinar stimulation correlate with improvement of awareness (Deutschová B et al, Brain Stimul, 2021) was published in Brain Stimulation thanks to a collaboration with the team of Fabrice Bartolomei from Service de Neurophysiologie Clinique, Institut de Neurosciences des Systèmes (INS), INSERM. In this study we focused on the neural mechanisms underlying alteration of awareness (AOA) and the effects of thalamic stimulation. Thalamic stimulation was shown to be effective in reducing AOA. I am a co-author of the article Resting-state phase-amplitude coupling between the human subthalamic nucleus and cortical activity: a simultaneous intracranial and scalp EEG study (Damborská A et al, Brain Topogr, 2021). Our results suggested that the subthalamic nucleus (STN) plays a role in the processes within the subcortico-cortical circuitries of the human brain and one of the important questions here was to assess whether unilateral DBS stimulation of the STN would be sufficient for treatment of Parkinson’s disease.
