Quality of Life of Seniors with Chronic Atrial Fibrillation

Abstract:

Cílem práce bylo zjistit kvalitu života seniorů s chronickou fibrilací síní. Dalším cílem bylo zjistit, zda je toto subjektivní vnímání kvalita života ovlivněné demografickými údaji a určit výskyt dalších přidružených onemocnění. Zjištěnou kvalitu života porovnat mezi oběma použitými dotazníky. Výzkumný soubor tvořilo 104 hospitalizovaných pacientů s diagnostikovanou chronickou fibrilací síní. Šetření proběhlo v období od měsíce ledna 2015 do listopadu 2015 v Městské nemocnici Ostrava na kardiologickém oddělení intermediární jednotky. Pro zjištění subjektivní kvality života seniorů s chronickou fibrilací síní byl použit specifický dotazník AF-QoL 18, který zkoumá kvalitu života pacientů s fibrilací síní a generický dotazník SF-12, který slouží ke zkoumání obecné kvality života. Do výzkumného souboru byli zařazeni pacienti, kteří byli v uvedeném období hospitalizováni, byli starší 65 let a bodové hodnocení kognitivních funkcí bylo 21 bodů a výše. Z výsledků výzkumu vyplývá, že kvalita seniorů s chronickou fibrilací síní, hodnocená pomocí dotazníků AF-QoL 18 a SF-12, není dobrá. Průměrné skóre hodnocení domén obou dotazník se pohybuje od 20-66% . Nejhůře dotázaní respondenti hodnotili oblast fyzické zdraví dotazníku SF-12 (20 %). Tento výsledek koreloval s hodnocením této oblasti dotazníku AF-QoL 18 (30 %). Nejlépe hodnocenou oblastí bylo prožívání bolesti. Respondenti uvedli, že je během 4 týdnů ovlivňovala středně. Dále byl zjištěn významný výskyt hypertenze u seniorů s chronickou fibrilací síní (89,4 %). 59,6 % respondentů mělo diagnostikovanou ischemickou chorobu srdeční. Obezitou v době výzkumu trpělo 55,8 % respondentů. Psychické zdraví bylo respondenty pomocí dotazníku AF-QoL 18 průměrně ohodnoceno 28 % a u dotazníku SF-12 25,5 %. Celková spokojenost s kvalitou života hodnocená dotazníkem AF-QoL 18 představuje průměrně 31 % a u dotazníku SF-12 26 %. Z těchto výsledků lze usoudit, že hodnocení kvality života seniorů s chronickou fibrilací síní je u obou dotazníků podobné.

Abstract:

The aim of the study was to determine the quality of life of older people with chronic atrial fibrillation. Another objective was to determine whether this is a subjective perception of quality of life affected by demographics and identify the presence of other comorbidities. Compare observed quality of life between the two questionnaires used. The research sample consisted of 104 hospitalized patients diagnosed with chronic atrial fibrillation. The survey was conducted in the period from January 2015 to November 2015 in Ostrava City Hospital cardiology department intermediate unit. To determine the subjective quality of life for seniors with chronic atrial fibrillation specific QoL questionnaire AF-18 was used, which examines the quality of life of patients with atrial fibrillation and a generic questionnaire SF-12, which is used to examine overall quality of life. The research group included patients who were hospitalized in the period mentioned above, were over 65 years of age and cognitive function scores were 21 points or higher. The research results show that the quality of seniors with chronic atrial fibrillation, where QoL was assessed by questionnaires AF-18 and SF-12, is not good. The average evaluation score of the two domains of the questionnaire ranges from 20-66 %. The worst area surveyed respondents rated physical health questionnaire SF-12 (20%). This result correlated with the evaluation of this field questionnaire Qol AF-18 (30%). The top-rated areas were in experiencing pain. Respondents said that during the four weeks it has affected them moderately. There was also a significant occurence of hypertension with elderly patients that suffered from chronic atrial fibrillation (89.4%). 59.6 % of respondents were diagnosed with coronary heart disease. At the time of the research, 55.8 % of respondents suffered from obesity. Mental health was evaluated with an average of 28 % to the questionnaire QoL AF-18 and 25,5% by the SF-12. Overall satisfaction with the quality of life shows an average of 31% in QoL AF-18 and 26% for the SF-12. From these results we can say that the quality of life for both questionnaires are roughly similar.
